After watching this video, I have mixed feelings that I could not describe in a word.
This incident could have been avoided if the workers and the employer had followed the
correct SOP to operate the machines. I felt very disgusted at the employer because of how
they neglected their workers' safety whereas the workers are the key to the company's success
because without workers there will be no production to generate income. It is undeniable to
say that we must blame the employer because we did not know whether the employer had
enacted the SOP or not. However, as you can see in the video the environment of the place is
not suitable for the machine to operate. For example, the worker needs to transfer the paper
from the first machine to the roller machine but the position of both machines are not suitable
because the first one is slightly higher than the roller one. Thus, the process will be harder
and uncomfortable because the worker needs to tiptoe a bit to reach the paper.
From the evaluation I have made from this video, there are two things that I can
identify. Firstly, the worker does not follow the SOP properly because in the video the worker
wears a long sleeve shirt. His clothes are suitable for office work not in the factory. In
addition, he did not use special shoes and face shields for the factory that had dangerous
machines. Besides that, the worker seems not to have enough experience and knowledge
about the machine as you can see his colleague does not know how to force stop the machine
when his friend got sucked into it. Secondly, the domino theory can be applied to this
incident. In the domino theory, an accident can occur only as a result of an unsafe act by a
person, or physical or mechanical hazards. It describes the whole situation in this video. But,
how it can lead to the domino theory? We can assume from the worker's reckless behavior
that maybe the worker has to achieve certain KPIs set by the employer or has a tight deadline.
Therefore, it may lead to pressure and cause reckless action.
After careful analysis has been made, the accident causation model is the immediate
cause. One of the types of incidents that can happen from this cause is being caught in or
between mechanical or other objects. Thus, it is proven that this incident is caused by the
immediate causes. Besides, the contributing causes to this incident are unsafe acts such as
protective equipment not being used, dangerous methods of handling, and dangerous
movement. Other than that, unsafe conditions also can be the contributing causes such as
hazardous housekeeping and improper dress for the job.
However there is some lesson that I have learned from this video, firstly as a worker,
you should not be reckless even if your employer puts pressure on you. Prioritize your safety
first than anyone else because if you are dead from an incident that happened at your
workplace the employer will always find your replacement but your life has no replacement
because it is what it is. Secondly, wear appropriate attire for the jobs assigned to us so that we
will not get into problems. The worker needs to use a tool to transfer the paper into the roller
machine by using tongs or a push bar. It is not suitable for the worker to transfer the paper
using his bare hand
Last but not least, in my opinion, to avoid this incident repeating in the future the
employer should install a kill switch on the machine or a sensor that can sense if it is the
material that the factory produces or not. If not then the machine will automatically stop in an
instance. Besides, the employer also has to be more careful when assigning the worker to
handle this type of machine. The employer needs to ensure that one of the workers has
enough experience and knowledge about the machine to guide the other workers if something
happens. In addition, the employer should have enforced the law to make sure all the
employees follow the proper SOP in the factory. This suggestion may reduce the risk of the
incident happening again in the future.
