AFFIDAVIT OF LOSS

I, MARY DIANE JAMISOLA SIOSON, of legal age, single, Filipino, with

residence at 23 ALPHA STREET BARANGAY BANGKULASI, NAVOTAS CITY, after
having been sworn to in accordance with law hereby depose and say that:

1. I was in possession of a cellular phone with Globe Prepaid Sim Card with
cellphone number 0915-765-3716.

2. That on December 2, 2022, while I was doing grocery errand, I had to make
an online transfer from my BDO account to my GCash number 09157653716.

3. Unfortunately, I forgot that my Globe number 09157653716 was not inserted

on my cellphone. I just realized it after wasn’t able to receive a text
notification of the successful transfer made.

4. I had placed my phone securely at the back of my phone including my sim

ejector tool but for some reason it got lost.

5. I took earnest efforts to locate the said nano sim card to all my whereabout
until I reached home but to no avail.

6. I execute this affidavit to attest to the truthfulness and veracity of the

foregoing facts, and for the purpose of securing another Globe Prepaid Sim
with the same number in place of the lost one, and for whatever purpose it
may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 3rd day of
