‫سنة ‪..‬‬
‫‪- Residence & address :‬‬
‫يعني هتسأل ا‪4‬ريض هو ساكن ف‪ C‬و هل هو مولود هناك اص> و = ‪ ..‬و اكتب‬
‫في الشيت هل ا‪4‬كان اللي بيكسن فيه ا‪4‬ريض‬
‫‪ = Rural‬ريفي‬
‫‪ =Urban‬حضر‬
‫عشان نعرف ال‬
‫‪Common infection in the patient's area‬‬
‫‪- Occupation.‬‬
‫‪- Special habits of medical importance :‬‬
‫‪ ::‬اهم حاجة ‪3‬‬
‫‪Smoking , alcohol abuse , drug abuse‬‬
‫و ممكن فيه دكاترة بيحبوا يسمعوا الشاي و القهوة ‪..‬‬
‫و لو مث> الحامل قالت ان جوزها بيدخن ‪ ..‬ف>زم تكتبوا في‬
‫الشيت انها‬
‫‪Passive smoker‬‬

‫) ‪- Menstrual history ( for females‬‬

‫هل الدورة منتظمة و ) ) ‪ ...‬بتيجي كل قد ايه ‪ ..‬و بتستمر‬
‫‪4‬دة كام يوم ‪ .‬ايه الكمية اللي بتنزل ) هتعرفها ‪4‬ا تسألها ع‬
‫عدد الحفاضات الصحية او الفوط اللي بتغيرها (‪ ..‬هل معاها‬
‫مضاعفات ‪..‬هل معاها نزيف حاد ‪ ...‬هل بتستعمل وسائل منع‬
‫حمل ‪...‬او لو هي كبيرة في السن تسالها الدورة انقطعت من‬
‫امتي ‪.......‬‬

‫‪b) Complain :‬‬

‫ايه اكتر شكوي اللي جابت ا‪4‬ريض النهارده ‪..‬ممكن اكتب واحدة او اتن‪C‬‬
‫بس‪...‬او مث> بيعمل‬
‫‪Follow up or vaccine administration … etc.‬‬

‫‪c) History of present illness :‬‬

‫تحليل كامل و مفصل للشكوي اللي جاي بيها ا‪4‬ريض ‪ ..‬و اعرف منه‬
‫امتي اخر مرة كان سليم ‪ ..‬و امتي بدأ يشتكي ‪ ..‬و اعمل تحليل لكل‬
‫ا)عراض اللي عنده ‪ ..‬و ‪4‬ا اكتب الشيت ابدا من بداية ا)حداث لحد‬
‫اليوم اللي جاه فيه ا‪4‬ريض للكشف ‪.....‬‬

‫‪d) History of investigations :: like lab tests , x-ray ,‬‬

‫‪Ultrasound … etc‬‬
‫‪e) Review of other systems :‬‬
‫يعني اتطمن ع باقي انظمة الجسم عند ا‪4‬ريض زي القلب و الصدر و‬
‫‪ ...‬ا‪4‬خ و ا)عصاب و العظام و الجهاز الهضمي و الكلية‬
 ‫عراض اللي ممكن يشتكي منها‬2‫يعني اسأله في كل نظام من دول ايه ا‬
... ‫اي حاجة‬..

f) Past history :
- Common diseases ( HTN , DM , Hepatitis C virus ,
bilharziasis , TB )

- Drug intake
‫ريض بياخدها بصورة مستمرة‬O‫دوية اللي ا‬2‫يعني ايه ا‬

- Drug allerge ‫ريض عنده حساسية من اي دواء زي كده‬O‫يعني هل ا‬

‫سبرين‬2‫ او ا‬T‫البنسيل‬

Penicillin , aspirin

- Previous operations
- Previous blood transfusion

g) Family history:
- Common diseases in family
- Consanguinity ‫ الزوج و الزوجة‬T‫صلة القرابة ب‬
- Genetic diseases
- Cancers

h) Socioeconomic history :
- Ventilation in house
- Number of rooms
- Job of husband , parents , …etc.

‫دي اسطمبة اساسية تنفع لكل انواع الشيتات‬

General ex.
a) Vital signs:
- Temperature :
Normally 36.5 – 37.2˚C
If oral = the same as you measure
If axillary = add 0.5˚C
If rectal = subtract 0.5˚C

- Respiratory rate:
✓ Normal in adult =12-20 cycle/min.
✓ Newborn(from birth till 2 months of age)= 60
✓ 2 months – 1 year = 50
 ✓ 1 year – 2 year = 40

- Pulse :
• Rate :: normally in adult 60-90 BPM (beat per minute
)
… Newborn =90-140

• Rhythm ( regular or irregular )

• Volume ( big , small , average=normal )
• Condition of vessel wall :: normally not felt ..if felt it
indicates atherosclerosis !‫تصلب الشراي‬
• Special character : the most common is water hammer
pulse which found in conditions of hyperdynamic
circulation (e.g pregnancy , anemia , thyrotoxicosis )

- Blood pressure : JNC 7 classification-- In adult….

In newborn : 80/50 mmHg till 4-5 years of age

b) 3 colours :
 • Jaundice : yellow : see sclera of eye
• Pallor : white : see tip of fingers , nails , mucous
membrane of lower lip
• Cyanosis : blue : see tip of tongue , tip of fingers ,
nails

c) General look :
• Orientation of patients to time , place , persons
• Mentality
• Body build … by BMI ( body mass index )
= weight ( in Kg ) / ( length in meter ) 2

• Eye …

d) Neck :
• Ex the carotid pulsation .. if present or not by your thumb
• Ex thyroid gland
By inspection see if there is any abnormality like swelling
, redness , pusltion , scars…
By palpation see if there is swelling , tenderness,….

• Lymph nodes .. as you know in skill lab

• Ex palatine tonsil … by inspection when the patient
open his mouth using tongue depressor ,, and by
palpation on its site in the upper part of neck
• Upper limb :: see skin if you find any abnormalities
… also see if there is clubbing of fingers
• Lower limb :: ex skin
See if there is edema or not …there are 2 types of
edema ..
✓ generalized ( due to cardiac , hepatic , renal or
nutritional conditions ).. edema present in both
legs starting from dorsal surface of foot
✓ localized ..edema present in one leg only
also in lower limb .. ex the presence of pulsation in
dorsalis pedis a. , posterior tibial a.

After one year --- weight = weight × 2 + 8

✓ head circumference
at birth…. 35 cm
1 year… 42.5
1.5 year … 45 cm
2 years … 47.5 cm

✓ mid-arm circumference ..normally 14 cm

if 12-14 .. subclinical malnutrition
if below than 12 .. clinical malnutrition
