RULES AND REGULATIONS

TRAINING AGREEMENT AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AGREEMENT

I, Reymark D. Este of Bestlink College of the Philippines hereby voluntarily agree to undergo Training in
Astoria Plaza for 15 days/month(s) from January 8 to January 22 at Astoria Culinary and Hospitality Institute [ACHI] in order for me
to acquire the knowledge and skills needed under the following terms and conditions:

a. That I shall pay as agreed with School Department Management and ACHI the equivalent fee for the duration of my training. That in case I decide not to
pursue my Training or stop in the middle of my training, ACHI shall not be obligated to refund to me the amount I have paid;

b. That I shall report and abide for my training based on agreed Training Days and schedule;

c. That I shall observe punctuality in reporting to the department I am assigned. I shall report for duty in proper uniform, with the prescribed nameplate and
be properly groomed at all times;
d. That ACHI is not required to provide my meal for the duration of my training; unless otherwise specified in a separate agreement;
e. That I shall observe 45 minutes break time, if assigned in operations & 1 hour break time, if assigned in the offices;

f. That I am aware that the hotel operates twenty-four (24) hours and that I may be asked to report on rotating shifts, including midnight shift. I am aware of
the risks and understand that this is necessary to complete my hotel training experience;

g. That I shall perform with care and diligence to the best of my ability whatever functions and responsibilities may be assigned to me during my Training
Program;

h. That I shall be answerable for any and all liabilities for damages to property or injury to third person which may be occasioned by my intentional or
negligent acts while in the course of my training;

i. That I shall renounce and waive any claim against ACHI for any injury that I may suffer particularly or pecuniary in the performance of my duties or
functions while under training;

j, That I shall abide by the rules & regulations of ACHI and shall comply with those policies imposed by the hotel. I shall not make any negative comments or
statements against said company, its officers and staff or its products Failure to do so, shall exclude me from further participation in the Training Program and
NO training certificate will be issued; and

j. That for the duration of my training, ACHI is not bound to pay me any compensation;

CONFIDENTIALITY
A Confidentiality of transactions, records, documents and any information pertaining to the business and its clients and their affairs will
be treated as such. No authorized disclosure or reproduction will be made any time during and after training.

TERMINATION OF TRAINING AGREEMENT

ACHI reserves the right to terminate this agreement upon violation of any of the hotel's rules and regulations. As such, no certificate of training shall be issued
thereto.

The following may also be causes for termination:

1. Stealing, exorting, or any attemp thereof
2. Dishonesty, forging, or tampering with records
3. Going out with employees and / or guests
4. Sleeping while on duty
7. Gambling
8. Use of prohibited drugs
9. Display of disorderly conduct
10. Bringing in and using of mobile phone or gadget
12. Carrying or possession of firearms & deadly weapons
13. Acts of lewdness, immorality, public display of affection, or
distribution of pornographic materials
14. Use of social networking, tools & emails for any illegal or
immoral purpose against Astoria-ACHI & Astoria Hotels & Resorts
15. Smoking inside the hotel/resort & while wearing training
uniform

5. Intoxication 11. Sexual harassment (including student trainee to

another student trainee) & any form of bullying
6. Destroying hotel properties or vandalism

In consideration of permitting my daughter/son ________________________________________ to participate in any way in the Training Activity, I for myself,
UNDERSTAND THE NATURE OF THE TRAINING ACTIVITIES AND MY CHILD'S EXPERIENCES AND CAPABILITIES AND BELIEVE MY CHILD TO BE QUALIFIED, IN GOOD
HEALTH, AND IN PROPER PHYSICAL CONDITION TO PARTICIPATE IN SUCH AN ACTIVITY, and Hereby RELEASE, DISCHARGE AND COVENANT not to SUE THE ACHI
TRAINING CENTER, the ASTORIA PLAZA HOTEL, MILLENNIUM PROPERTIES AND BROKERAGE, INC. their RESPECTIVE ADMINISTRATORS, DIRECTORS, AGENTS,
MANAGERS or EMPLOYEES on which the ACTIVITY takes place, from ALL LIABILITY, CLAIMS, DEMANDS, LOSSESS OR DAMAGES ON ANY ACCOUNT CAUSED OR
ALLEGED IN WHOLE OR IN PART and I further agree that if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I,
or any one on my behalf makes a claim on any of the RELEASE, I will INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASES from any litigation
expenses, attorney's fees, loss, liability, damage or cost which may incur as result of such claim.

I HAVE READ THIS AGREEMENT, FULLY UNDERSTOOD ITS TERMS, AND HAVE SIGNED IT FREELY AND WITHOUT INDUCEMENT OR ASSURANCE OF ANY
NATURE AND INTEND TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
