Incident Reporting

KEY TERMS
environmental restraint a modification to the space around a client that limits mobility
hazard a source of danger; a possible cause of an incident
incident report a paper or electronic form that must be completed following an incident to summarize the event,
the circumstances and actions leading up to the event, and the way in which the event was resolved
incident an unusual, unintended event that occurs at a workplace and causes harm, or has the potential to cause
harm, to clients, staff, or visitors
monitoring device an alarm that sounds when the client moves or tries to exit
physical restraint prevents clients’ access to their own bodies and/or limits their movement
sharps container a puncture-proof receptacle for the safe disposal of sharp items like razors and needles

TEACHING STRATEGIES AND SUGGESTED STUDENT ACTIVITIES


● Ask students to discuss the feelings they might experience in the case of a fire. How might a client who is
unwell or has limited mobility feel or react? How might this impact the role and actions of the personal care
provider? (LO2)

● Divide the learners into groups to discuss different types of disasters, and then ask them to brainstorm what
their roles, as personal care providers, would be in preventing them. (LO3)

● Apply different restraints to a handful of volunteers, and ask them wear them during class time. Use Posey
restraints, large mitts, transfer belts, belt them into a chair, etc. (LO5)

● Show and review a completed incident report. Provide students with a copy of a blank incident report, and
then have them document an incident scenario. This can be done in pairs or individually. (LO9) Examples
of incidents are as follows:

– a fall

– a burn with hot coffee

– a skin tear from bumping against a table

– a medication not administered on time
