to participate in the 2023 DIVISION TRAINING FOR SCHOOL PAPER ADVISERS AND CAMPUS JOURNALISTS on
JANUARY 5 – 6, 2023 at PALAMIS ELEMENTARY SCHOOL, Alaminos City, Pangasinan.

I have considered the benefits that shall be derived from the participation of my son/daughter in the
said training with the understanding that the necessary safety measures/precautions shall be taken.

However, I shall not hold the Balayang Elementary School and Staff liable if, after such safety
measures/precautions taken untoward incidents happen.
