Overseas/ Mainland Attachment Programme
I visited two eye hospitals, the Oxford Eye Hospital and the
Moorfields Eye Hospital, during the Scholarship period. Working
with a group of clinicians specialising in retinal surgery at the Oxford
Eye Hospital, I learnt more about various vitreoretinal pathologies and
surgical techniques. What piqued my interest the most, however, is
their work on inherited retinal diseases, which represent a spectrum
of diseases that constitutes a major cause of blindness worldwide.
There are more than twenty phenotypes, depending on the underlying
pathology. However, they may present with similar clinical features,
especially towards the latter stages, with irreversible characteristics
like cell death and retinal atrophy. To further add to the diagnostic
challenge, the genetic makeup of these diseases has much inter-
allelic and intra-allelic overlap and variability. Owing to such clinical
and genetic heterogeneity, there remain significant unmet challenges
in terms of diagnosis and management.
The pandemic has also changed the landscape for medical students
worldwide. The incorporation of telemedicine into the curriculum has
inadvertently developed our digital literacy and our awareness of data
confidentiality and patient safety. Through virtual education, I had the
opportunity to participate in the neuro-ophthalmology conference
held by the Singapore National Eye Centre (SNEC). The masterclass
included clinical skills transfer sessions, whereby ophthalmology
residents would perform the clinical examination on real patients,
followed by instant feedback on areas for improvement. I was able
to reflect on my own examination techniques and the diagnostic
approach I use to reach the most probable diagnosis.
24
Through these clinical experiences, I learnt what it is like to work
in the National Health Service (NHS). Despite having done a few
attachments before I applied to medical school and as a medical
student, gaining more clinical experiences meant that I could
fully appreciate the similarities and differences between the two
healthcare systems. Notably, the differences in the IT systems
demonstrated the importance of technological infrastructure in
determining efficiency, costs, and staff morale. The NHS computer
system, previously dubbed one of the biggest IT failures, has since
improved. However, I noticed that each facet of patient care needs
to be brought up separately, which means more waiting time for the
software to load and is often the cause of much frustration amongst
doctors. In contrast, in Hong Kong these are embedded within one
system with seamless transition between different facets.

conduct research alongside my future clinical duties.
Since genetics is typically taught at rather senior levels (usually in the
secondary school biology curriculum), I had to think of simple and
creative ways to engage children and stimulate their interest in the
topic. I decided to borrow the concepts of Mendelian inheritance in
humans to create an interesting workshop called ‘Alien Genetics’ (
遺傳大揭秘——外星人也有基因?!) Even though the fifth wave of
COVID-19 in Hong Kong meant the workshop could only be held via
Zoom, it attracted more than 50 participants. Many of them asked
inspiring questions not just about the content of the workshop, but
also about careers in biology and medicine. It felt truly amazing to
know that I could be playing a part in shaping the future of bright
young minds and inspiring them to pursue careers in biomedicine.
Ever since my time tutoring students in high school, I have been
interested in educating our future generations, and this service
project really consolidated this interest. One day, I hope to join the
ranks of the teachers and professors I have always admired and pass
on the light of knowledge to future generations.

In the clinical realm, I was mainly immersed in the vitreoretinal team that
dealt with the posterior part of the eye. I observed a variety of intriguing
