I, CRIS FREDRICH T. ANDALIZA, am writing this letter to authorize my sister,

BRYLYN T. ANDALIZA, to claim the rice allowance on my behalf.

Due to unavoidable circumstances that prevent me from personally claiming the rice
allowance provided by City Government of Borongan, I hereby designate my sister as my
authorized representative for this matter.

I trust that she will handle this task responsibly and complete all necessary procedures
required for claiming the rice allowance. She will present her identification and any other
relevant documents to validate her identity and claim the rice allowance on my behalf.
