level 3 K5 CUB Building will be carried out in safely manner and to follow the standard lifting
procedure being implemented.

4.0 Logistics Plan

4.1 Access

 Access will indicate in the Lifting plan and get the approval from Exyte 1 day before
site is ready to move in

 All worker, project management team, material or equipment and machinery delivery
must be entering from the site entrance and transporter detail must be provided to
security team 1 day in advance.

 All visitor and vendor which are entering the site should apply permit for temporary
badge from the security team.

4.2 Receipt of Materials

 Lifting plan will incorporate the traffic control plan during lifting activity.

 Adequate protection will be provided for the material/access route to ensure that
there is no damage during the transportation process.

 Extension service platform structure members will be marked (labelled) at factory to


ease the identification of the structure members upon reaching the site.

 Latest construction drawing shall be prepared and ready onsite upon receiving of
materials.

 During the loading and unloading the driver of the trailer shall switch off the engine
and move to a safe area.

 The supervisor shall ensure compliance to traffic management procedures.

4.3 Handling

 Lifting activity will be conducted as per approved Lifting Plan and construction
drawing.

 Task lighting will be provided by ZSCECM if the activity will extend beyond 1800
hours/ when is necessary, subject to the environment.

 Ensure all the handling of materials to be supervised and handled in a manner to


prevent damage.

 All the worker involved must have proper PPE.

 Read labels and Material Safety Data Sheets (MSDS) before storing chemicals or
flammable materials. Smoking is strictly prohibited on site. Non-compatible materials
must be segregated in storage.

 All spills of flammable/combustible materials shall be immediately clean up.

 Work will be carried out in compliance with safety requirements and legal regulations.

4.4 Logistics / Mobilization Arrangements for Hoisting

 Hoisting is to be checked overall repeatedly to prevent accidents.

 Fulltime monitor by Lifting Supervisor, Rigger and Signalman (Competent).

 Provide lifting plan with crane data / certification.

 Lifting activity and mobilization shall be done by trained person and use correct tools/
equipment as per agreed by Exyte management.

 Carry out site inspections and plan approval from Exyte officer 2-3 working days
earlier before start any erection at site

 The application for road occupation for hoisting must be made 1-2 days in advance.
During hoisting, barricades shall be set up in the hoisting area and maintained.
Personnel shall not pass during hoisting

 The crane must be accepted by Exyte, and enter the work site only after getting
approval. Crane operators and signalmen must have relevant certificates, and non-
professionals are not allowed to operate

 25% margin for crane selection (safe load)

 Crane to be barricaded to prevent unauthorized use by other party and no materials


shall be leave at work area after the daily site work completion.

 Check logistics plan, access road, ground condition for unit delivery and mobile crane
parking/hoisting task

4.5 Storage of materials

- All the material should be stored in a proper manner with barricading and appropriate
signages.

- All the materials stored shall be covered up with no direct contact with ground.

- All the material shall not block emergency access or equipment and all the
passageways must be kept clear of obstructions and slip, trip and fall hazards. A 36-
inch clearance shall be maintained around emergency equipment and the emergency
equipment shall be clearly marked.

- Keep storage area free from accumulated material that may causes slips, trips, falls
or fires or that may contribute to harboring pests.

- Stack material no more than 1.5m if handled manually and no more than 2m fi using
a forklift and remove all nail from the used lumber fore stacking it.

5.0 Safety Procedure & Control

5.1 Job Safety & Healthy Analysis

Job Safety & Health Analysis (HIRARC) shall be conducted on all hazardous activities at
project site. The hazards in a particular activity and the control measures n implemented in
the HIRARC shall be briefed to the workers during safety induction and toolbox meeting

5.2 Information to Personnel

 All workers employed in the project site must have valid CIDB green card.

 All new workers and staff must attend the Safety Training or Safety Induction Course before
commencing works at site and attendances shall be recorded in an induction checklist.

 All the personnel will be briefed by the safety supervisor.

 All site personnel shall wear proper Personnel Protective Equipment (PPE):
Safety helmets, google, glove, safety vest, safety boots and Wellington Boots with steel toe.

 Monthly safety meetings shall be held with all the contractors’ related-to-works.

 All workers must attend the daily toolbox meetings held at site and attendances shall be
recorded.

 Traffic signal man to execute the traffic management plan & control the traffic flow during
the execution works on affected areas.

5.3 Safety Arrangements and Equipment

 Driver or operator should follow the safe operating instruction laid out in the operation
manuals of the respective transport vehicles or machineries.

 All the trailer or lorry entering the site shall be guided by a banksman with a speed limit of
15 km/h and switch on the hazard lights whilst escorting the incoming or outgoing vehicles.

 During unloading a tire stopper must be in place, barricade the area and put up signage.

 All the trucks and trailers should pass through the wheel washing bay before leaving the
site and workers will be assigned to carry out road cleaning with water if necessary.

 Cranes must not be left unattended whilst suspending a load. When not in used and left
unattended, crane cabs and compartment must be kept closed with controls or power pack
locked off and the vehicles parked without obstructing access.

 No machinery shall be operated without the guidance of banksman.

 No construction activity will be carried out during heavy drain.

 Site access and external road to maintain in clean condition at all time.

5.4 Accident Prevention Safety Procedures

5.4.1 Installation and erection of extension service platform

 Area should be barricade adequately with warning barriers prior to any lift.
 All equipment shall be checked for good conditions before commencing the
work
 Unauthorized persons shall be excluded from the area.
 When lifting the personnel shall not remain in the lifting equipment work area.
 Adequate caution boards and waring sign shall be provided
 Only trained and authorized personnel shall operate the hydraulic lifting
device
 Observance of the lifting equipment capacity.
 lectrical installation and equipment shall confirm to standards.
 Application, maintenance, storage, of electric and oxygen cutting welding
units shall confirm to applicable standards.
 Do not stand under suspended loads
 On completion of job, the area shall be cleared of any materials and kept
clean and tidy.
 Hazard analysis for hazardous operation shall be carried out prior to
commencing the works.

5.6 Risk Assessments

5.6.1 HIRARC - the propose of HIRARC are as follow: -

- To identify all the factors that may cause harm to employees and other (the
hazards).

- To consider what the chances are of that harm actually be falling anyone in
the circumstances of a particular case and the possible severity that could
come from it (the risk).

- To enable employers to plan, introduce and monitor preventive measures


to ensure that the risk are adequately controlled at all times.

5.6.2 Likelihood of an occurrence

This value is based on the likelihood of an event occurring. You may ask the question “How many
times has this event happened in the past?”. Accessing likelihood is based worker experience,
analysis or measurement. Likelihood level range from “most likely” to “inconceivable”

5.6.4 Risk assessment

- Risk can be presented in variety of ways to communicate the result of


analysis to make decision on risk control. For risk analysis that uses
likelihood and severity in qualitative method. Presenting result in a risk
matrix is a very effective way of communicating the distribution of the risk
throughout a plant and area in a workplace.
