To whom it may concern,

Please accept this letter as a notice of my resignation as a Patient Services Representative II

from Conifer. My last day of employment will be on July 31, 2023 with a 30-day render.

I have thought about leaving for a few months now and I understand that this notice is very
sudden. Due to the several issues that I’ve encountered regarding my salary and the disputes
that I’ve raised few months back without any resolutions until today, I have decided to move on
and continue my career with another company.

Although there are issues that I’ve experienced, it has been a great pleasure working with you,
with the team and the management. I have learned many things in 2 years of staying with the
company. I wish you, and Conifer more success in the future.
