student to have their practice in Dance Showdown in HOPE 3 (Dances) on _________________

School Curriculum as their PERFORMANCE TASK.
We deeply believe that his/her participation to the above-mentioned event is beneficial to his/her
development as a student and as an ordinary individual as a whole. We further adhere to the
policy that all necessary precautionary measures will be observed by the accompanying
representatives from the school and that we do not hold the school nor its representative
responsible for any untoward incident that may happen beyond their control during the said
event.
