Agreement Letter

RE: Scholarship Renewal - Academic Year 2023-2024, Second Semester

This document serves as a waiver agreement outlining the terms and conditions for the renewal of
your scholarship for the second semester of the academic year 2023-2024. We are pleased to extend
this opportunity to support your ongoing educational pursuits.

As per our agreement, you are eligible to renew your scholarship under the following conditions:

1. Completion of 180 hours of duty as an online student assistant.

2. Maintenance of a good attendance record in your Program Outcome Audit (POA) review.

You acknowledge and agree that failure to meet these specified requirements will result in the
revocation of the discounted amount in your scholarship, and you will be held liable to pay the full
tuition fee amount for the school year 2023-2024.

By signing this document, you indicate your understanding and acceptance of the terms and
conditions outlined above. Your compliance with these conditions is crucial to retaining the benefits
of the scholarship for the specified semester.

Please sign below to indicate your acceptance and return a copy of this signed waiver by January 5,
2023.
