AFFIDAVIT OF CHANGE ADDRESS

I, ALEX JOSE G. BETINOL, of legal age, Filipino, married to ELAINE

ABELLON BETINOL, and with residence at Bonifacio Street, Poblacion District 1,
Dauin, Negros Oriental, Philippines, after having been duly sworn to in accordance with
law, do hereby depose and state: That

1. I am a respectfully informing and requesting the Philippine National Police,

Civil Security Group, Firearms and Explosives Office, and any other offices
for the amendment and change of my home address from:

185 Villa Fortunata, Batinguel, Dumaguete City

To my present address:

Bonifacio Street, Poblacion District 1, Dauin

2. I, together with my family transferred to our new residence last December

2019;

3. I am currently processing my renewal of License to Own and Possess Firearms

(LTOPF); and

4. I am executing this affidavit of Change of Address in order to amend and

change my residence address indicated in my Firearms Licensed to my new
address and for whatever legal purpose this may serve.
