AFFIDAVIT OF LOSS

I, JULIE ANNE B. MACATLANG, of legal age, Filipino citizen,

married and a resident of 111 East Villa Capri Circle, Apartment D,
Deland, Florida, 32724, U.S.A., after being duly sworn to in
accordance with law, hereby depose and state:

1. That I am a bona fide holder of a college diploma duly issued

by St. Scholastica’s College, Metro Manila, Philippines, for
having graduated with the course of (AB OR BS) in the year
____.

2. That sometime on December 15, 2023, I noticed that the said

document was missing while moving my things to our new
apartment;

3. That despite diligent efforts exerted for the purpose of locating

and recovering it, all said efforts proved futile as I failed to find
and recover the diploma up to the present;

4. That said diploma has not been revoked nor been cancelled
for any reason; and

5. That I am executing this Affidavit for the purpose of attesting

to the truth of the foregoing statement, and also for all legal
intents and purposes.
