MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This MEMORANDUM, entered by and among:

The APAYAO STATE COLLEGE is a state institution of higher learning duly

organized and created under laws of the Republic of the Philippines, with principal office at
ASC Compound, San Isidro Sur, Luna, Apayao, represented by its OIC-President, DEMETRIO
P. ANDUYAN, JR., Ph.D. CESE, and hereinafter referred to as ASC;

AND

The ____________________________________ with

principal office at ________________

______________________and represented in this agreement by its Municipal

_____________________, and hereinafter referred to

as_____________.

WITHNESSETH:

WHEREAS, the pursuit for quality education cannot be solely shouldered by a single
educational institution. Instead a partnership should be affirmed with the stakeholders
where the resources are equitably shared to achieve its vision, mission, goals and
objectives as set forth in their corresponding mandates;

WHEREAS, both parties have mutual and typical needs, beneficial to each other and with
common felt need to improve the quality of education;

WHEREAS, ASC aims to enhance its manpower capability and keep itself abreast to the
emerging trends in Agriculture profession;

WHEREAS, ASC is a higher learning institution offering Bachelor of Science in Agriculture

Program for prospective agriculturist for agricultural extension works, researches,
instruction, and agricultural production, and other programs namely, Forestry, Teacher
Education, Business Administration, Hotel and Restaurant Management and Tourism, and
Information Technology;

WHEREAS, ASC recognizes the capability of the Municipal Agriculture Office of the Local
Government Unit of Flora, to provide the actual-work knowledges, skills, attitudes, and
networks to agriculture students and other student-trainees in their On-the-Job Training
(OJT);

WHEREAS, ___________ has the capability in providing the pertinent involvement

for fulture agriculturist and other student-trainees in the fields of agricultural extension,
agricultural production, and office work.
NOWTHEREFORE, for and in the consideration of the foregoing premises, both ASC and
_________ hereby agree on the following terms and conditions of this Memorandum
of Agreement;

FOR ASC:

1. Submit a list of qualified students for the On-the-Job Training to the

____________ a week before the practicum program commences;
2. Shall ensure that participation by agriculture students and student-trainees shall be
voluntary;
3. Coordinate with the office of the Municipal Mayor and the Municipal Agriculture
Office before the OJT students enters the municipal office; and
4. Avail of the services of the human resources to serve as resource speakers,
consultants and other services offered by the College.

FOR Participating Agency:

1. Allow the agriculture on-the-job trainees to be deployed in the municipal agriculture

office;
2. Provide relevant learning experiences to student-trainees during their stay in the
municipal agriculture office and community if deemed necessary;
3. Provide the agriculture students and other student-trainees access to information
regarding the office if deemed necessary;
4. Provide the agriculture students and other student-trainees with the best mentors
to enhance their experience and prepare them as agriculture professionals;
5. Report to proper authority in ASC any untoward incident that a student-staff or
student-trainee encounters during his/her stay in the LGU;
6. Observe proper protocols for providing actions for student-agriculturist or student-
trainees’ violation in their agriculture extension work, office work, or on-the-job
training; and
7. Submit performance evaluation reports of student-agriculturist and other student-
trainees.

LIABILITY

The School, its administrator and faculties exercising authority and supervision over
student-agriculturist and student-trainees undergoing training may be held responsible for
the students’ acts.

Each party shall answer for loses and damages arising from any accident, act or omission
directly attributable to its fault or negligence which may cause death or bodily injury to any
person, or loss or damage to property, by or on account of the performance of the
respective obligations by the parties pursuant to this Agreement.

In case of COVID-19 infection or exposure, cost for the quarantine, isolation or treatment
shall be for the account of the ASC.
EFFECTIVITY OF THE AGREEMENT

The terms and conditions of this agreement shall be effective upon the approval and
signing of both parties. Provisions of this agreement may be modified, altered or
supplemented subject to the mutual agreement of both parties.
