In connection with your contingent offer of employment, please be advised that Sanmina Corporation may use
a consumer reporting agency to obtain a consumer report (“Report”) as part of its hiring process. Further,
please be advised that if you are hired, to the extent permitted by law, Sanmina Corporation may obtain further
Reports from a consumer reporting agency so as to update, renew, or extend your employment.

Reports provided by a consumer reporting agency may include information regarding your character, general
reputation, personal characteristics, mode of living, and credit standing.

If a consumer report is requested, you may request a copy of the federal Fair Credit Reporting Act Summary of
Rights as well as information regarding the nature and scope of any requested investigative consumer report.

DISLOSURE OF INTENT TO OBTAIN A CALIFORNIA INVESTIGATIVE CONSUMER REPORT

In connection with your contingent offer of employment, Sanmina Corporation (“the Company”) will be
requesting from First Advantage Corporation located at 1 Concourse Parkway NE, Suite 200, Atlanta GA
30328 Phone Number: 1800-888-5773 an investigative consumer report for employment purposes (i.e.
evaluating you for employment, promotion, reassignment or retention as an employee). The Consumer
Reporting Agency’s website address where its privacy policy can be found is https://www.fadv.com/privacy-
policy.aspx.
Under California law, the Report(s) requested are considered an investigative consumer report and it may seek
information regarding your general reputation, character, personal characteristics, and mode of living.
The specific nature and scope of the investigation sought by the Company is as follows: criminal history
consistent with applicable federal and state law; education verification; employment verification; and social
security match.
You have the right to inspect visually the files concerning you maintained by an investigative consumer
reporting agency during normal business hours and upon reasonable notice. The inspection can be done in
person if you appear in person and furnish proper identification; you are entitled to a copy of the file for a fee
not to exceed the actual costs of duplication. You are entitled to be accompanied by one person of your
choosing, who shall furnish reasonable identification. The inspection can also be done via certified mail if you
make a written request, with proper identification, for copies to be sent to a specified addressee. You can also
request a summary of the information to be provided by telephone if you make a written request, with proper
identification for telephone disclosure, and the toll charge, if any, for the telephone call is prepaid by or
directly charged to you. You further understand that the investigative consumer reporting agency shall
provide trained personnel to explain to you any of the information furnished to you; you shall receive from the
investigative consumer reporting agency a written explanation of any coded information contained in files
maintained on you. “Proper identification” as used in this paragraph means information generally deemed
sufficient to identify a person, including documents such as a valid driver’s license, social security account
number, military identification card and credit cards.

AUTHORIZATION AND CONSENT TO OBTAIN CONSUMER REPORT FOR EMPLOYMENT
PURPOSES UNDER THE FEDERAL FAIR CREDIT REPORTING ACT AND/OR CALIFORNIA
INVESTIGATIVE CONSUMER REPORT FOR EMPLOYMENT PURPOSES
Consistent with the written Disclosure of Intent to Obtain [Consumer Report For Employment Purposes Under
the Fair Credit Reporting Act] [Investigative Consumer Report For Employment Purposes Under the Federal
Fair Credit Reporting Act] and the written Disclosure of Intent to Obtain a California Investigative
Consumer Report, both dated __________ and provided to me, I hereby consent to and authorize
Sanmina Corporation (the “Company”) to obtain an investigative consumer report under the Fair Credit
Reporting Act and/or California investigative consumer report (“Reports” ).
If hired, and to the extent permitted by law, I also consent to the Company obtaining further investigative
consumer reports for employment purposes pertaining to me, as described in the disclosures, from the
identified investigative consumer reporting agency at any time during my employment with Sanmina
Corporation. I understand that any future reports sought by the Company will be consistent with the specific
nature and scope of the requested investigative consumer report described in the Disclosure to Obtain a
California Investigative Consumer Report that I received or, if it is not, that I will receive new disclosures and
a new authorization to sign.
If an investigative consumer report, as defined under the federal Fair Credit Reporting Act, is requested, I
understand that I may request a copy of the federal Fair Credit Report Act Summary of Rights as well as
information regarding the nature and scope of any such requested investigative consumer report. I also confirm
my receipt of a copy of the Summary of Rights with the Disclosure of Intent to Obtain Consumer Report or
Investigative Consumer Report for Employment Purposes Under the Federal Fair Credit Reporting Act.

I understand I have the right to request a copy of my investigative consumer report/consumer report/consumer
credit report pertaining to me obtained by the Company from the consumer reporting agency by checking the
box below. If the box is checked, I understand that any requested report will be mailed directly to me without
cost by the investigative consumer reporting agency at the same time the report is sent to the Company.

I wish to receive a copy of the investigative consumer report/consumer report/consumer credit

CALIFORNIA CONSUMER CREDIT REPORT DISCLOSURE AND AUTHORIZATION

Disclosure
Sanmina Corporation’s request for a consumer investigative report from First Advantage Corporation located
at 1 Concourse Parkway NE, Suite 200, Atlanta GA 30328 Phone Number: 1800-888-5773 for employment
purposes (i.e. evaluating you for employment, promotion, reassignment or retention as an employee) includes a
request for information pertaining to your credit worthiness, credit standing or credit capacity, which
constitutes a consumer credit report under California law. Sanmina Corporation seeks a consumer credit report
on the basis the position you are being evaluated for is:
__X_ A managerial position which qualifies for the executive exemption from the
overtime requirements of California state law under Wage Order 4-2001.

___ A position in the state Department of Justice.

___ A position as a sworn peace officer or other law enforcement position.

___ A position for which the information contained in the report is required by law
to be disclosed or obtained.

___ A position that involves regular access, for any purpose other than the routine solicitation and
processing of credit card applications in a retail establishment, to all of the following types of
information of any one person:
(A) Bank or credit card account information.
(B) Social security number.
(C) Date of birth.

___ A position in which the person is, or would be, any of the following:
(A) A named signatory on the bank or credit card account of the employer.
(B) Authorized to transfer money on behalf of the employer.
(C) Authorized to enter into financial contracts on behalf of the employer.

___ A position that involves access to confidential or proprietary information, including a formula,
pattern, compilation, program, device, method, technique, process or trade secret that (i) derives
independent economic value, actual or potential, from not being generally known to, and not being
readily ascertainable by proper means by, other persons who may obtain economic value from the
disclosure or use of the information, and (ii) is the subject of an effort that is reasonable under the
circumstances to maintain secrecy of the information.

Authorization

Consistent with the disclosure above, I hereby consent to and authorize Sanmina Corporation to obtain a
consumer credit report (“Report”), as defined under California law, from the Consumer Credit Reporting
Agency identified above.

By checking this box, a copy of the consumer credit report will be sent to
you at no cost by First Advantage Corporation at the same time it is sent to
Sanmina Corporation.
