KNOW ALL MEN BY THESE PRESENTS:

I, NATHANIEL JOSEPH BARRAMEDA AROSA, of legal age and a Filipino citizen, single,
and a resident of Unit 046-I Bautista Apartment, 1st Reyville Subd., Habay 2, Bacoor City of
Cavite Philippines, after having been sworn in accordance with the law, hereby depose and
state the following:

1. I am the owner of the Mobile Phone/Gadget with the following details and
description;

2. I have recently discovered the said item irretrievably lost and can no longer be
found despite diligent efforts to locate the same since it was snatched/grabbed to
me on my way home inside the Jeepney;

3. I declare that the said mobile phone/gadget is now lost beyond recovery;

4. I execute this affidavit to attest to the truth of the foregoing.
