AFFIDAVIT OF GUARDIANSHIP

I, JOVITA YAP DUMLAO, of legal age, single, Filipino and a resident of 2407 Topacio
San Andres Bukid, Sta. Ana Manila, Philippines after having been duly sworn to in accordance with
law, do hereby depose and state:

1. That I am the Aunt of the following minors named

 YARAH ALEXA DUMLAO GUILLERMO, who was born on Jan. 18, 2007
 LATEEN IMIGEN DUMLAO GUILLERMO, who was born on Sep. 16, 2011,
 SAM KEITHLEEN DUMLAO GUILLERMO, who was born on Nov.. 14, 2017
to his/her biological parents JOSE AZUR GUILLERMO JR. And MARGARITA DUMLAO
GUILLERMO;

2. That the said child has been under my guardianship and custody for one and a half month
because their parents will go to the United States of America to process the U.S Passport of their
Children;

3. That I am the only one been providing/supporting her basic needs; And,

4. That I am executing this Affidavit to attest the truthfulness of the foregoing statements
and for whatever legal purpose this may serve;
