AFFIDAVIT OF PAYMENT

I, BERNARDO M, CRISOSTOMO, of legal age, Filipino, married and with

postal address at Abordo Subdivision, Brgy. San Jose Puerto Princesa, being
first duly sworn in accordance with law, depose and state that:

1. That I am the owner / proprietor of BMC GROCERY STORE, located at

Abordo Subdivision, Purok Masikap, San Jose, Puerto Princesa City,
Palawan;

2. That, I had sold to IGLESIA NI CRISTO, Two Thousand One Hundred

Pieces (2,100) of Mega Sardines, Two Thousand One Hundred Pieces
(2,100) HOMI Chicken Noodles with a total amount of Sixty Eight
Thousand Two Hundred Fifty Pesos ( P 68,250.00) on April 30, 2023;

3. That, the IGLESIA NI CRISTO paid the corresponding amount indicated

above.

4. I am executing this affidavit to attest to the truth and veracity of the

foregoing facts and for whatever legal purpose it may serve.
