 1. RESEARCH OF MEDICAL STATEMENT.

What is a Medical Statement?


Medical records are contained in a medical statement, which is a formal and legal document. All
medical records are kept private and secret; in fact, physicians are not permitted to reveal any of them
because they are private and personal. When there are activities with potential risks, a medical statement
is frequently utilized and necessary.

 MAKE A LIST OF MEDICAL STATEMENTS THAT YOU KNOW OR SEE ON THE INTERNET.

I haven't found some medical statements from the internet with some names and examples, but I have found some examples
of medical statements templates, so these are the ones that I will use to determine if they are facts or opinions. Some of the
Medical Statement I have seen on the internet are;

 ANALYZE IF THE STATEMENTS ARE STATING A FACT OR NOT

Since medical statements are confidential, I cannot find some of the statements on the internet with
names and actual examples. So, I think, based on my research and these templates, all of these are
facts. Though I think it's obvious that medical statements should be all facts and contain the medical
records of a patient or participant, I think it is all a fact because it has the parts of a medical
statement wherein, we all see there are spaces for the patient's or participant's information such as
name, address, date of birth, contact details, and social security number. The physician's
information such as license number, name, position, address, and contact details, the list of
diagnostic tests that were conducted or performed on the participant, and the signatures of the
physician and participant as a declaration that all the information provided is true and up to date.
Lastly, as a reminder, a medical statement form is only permitted or legitimate if and only if the
physician's and participant's signatures are appended as a declaration that all the information on
the form is accurate.

1. WHAT ARE THE MEDICAL STATEMENTS THAT ARE NEW TO YOU?

All of these medical statements are new to me, but there are 2 that are even more new to me: the medical
impact statement and the medical source statement, which are both written testimony by a health care
professional about a patient’s medical situation. It highlights any functional limitations of a person as a
result of a recent diagnosis.

2. WHERE DID YOU GET THE MEDICAL STATEMENTS THAT YOU LISTED?

I got the medical statements that I stated from the internet using Google for my research.

3. HOW DID YOU CLASSIFY IF IT’S A FACT OR AN OPINION?

I determined whether it was a fact or an opinion by determining that facts are objective, and opinions are
subjective, that facts are consistently repeatable in terms of measurement and observation with empirical
senses, and that opinions are not. Fact has been subjected to scientific scrutiny, is objective, verifiable, and
has been demonstrated to be correct.

4. WHY DO WE NEED TO BE AWARE OF THE VALIDATION OF THE


STATEMENTS THAT WE GET?

We must be aware of the validation of the statements we receive since it guarantees that a patient's
assessed needs are addressed completely, and it also ensures that you have objective evidence that specified
requirements are completed.
