GUIDELINES ON HOW TO CONSTRUCT QUESTIONNAIRE

1. The quetionnaire should have 4 sections:

Section A: Demographic Data

Section B: Factors / Reasons

Section C: Effects / Consequences

Section D: Suggestions / Recommendations

2. The questionnaire should contain 15 questions.

* Not restricted to each section to have 5 questions. Can have more than 5 questions depend on
your topic.

3. The questionnaire should be used to interview 50 respondents limited to UniKL Business School
students ONLY.

4. Section A: Demographic Data (ONLY HAVE 4 QUESTIONS)

5. EXAMPLE Section B: Factors affecting poor time management (depends on your topic)
