    WORKSHOP to be held on September 15-17, 2023 at Senator Ninoy Aquino National High
    School, Brgy. Kulaman, Senator Ninoy Aquino, Sultan Kudarat, Philippines.

    I have considered the benefits that my child will obtain from his participation provided that
    due care and precautions will be observed to ensure the comfort and safety of my child and
    that DepEd employees and personnel may not be held responsible for any untoward incident
