AFFIDAVIT OF LOSS

I, CHARMAINE VENUS M. UY, Filipino, of legal age, single, and

with residence address at #9 Rustia St., E. Rodriguez Avenue, Cubao,
Quezon City, after having been duly sworn to in accordance with law,
depose and state that:

1. I am the owner and holder of an AUB Payroll Account ATM

Card (“ATM Card”) which was issued in connection with my employment
at TSD Global.

2. I used to place the ATM Card inside my coin purse.

3. Sometime on 15 May 2018, I looked for the ATM Card inside

my coin purse but realized that it was already missing.

4. I exerted diligent efforts to locate the ATM Card. However,

efforts to do so proved futile, as it could no longer be found.

5. I am executing this Affidavit to attest to the truth of the

foregoing and in support of my request for a replacement ATM Card.

IN WITNESS WHEREOF, I have hereunto set my hand this 22 nd day

of May 2018 in Taguig City, Metro Manila, Philippines.

I, ROSALLIE BOGNOT MENDOZA, Filipino, of legal age, single,

and with residence address at 210-B Roldan St., Hagonoy, Taguig City after
having been duly sworn to in accordance with law, depose and state that:

1. I am the owner and holder of an AUB Payroll Account ATM

Card (“ATM Card”) which was issued in connection with my employment
at TSD Global.

2. I used to place the ATM Card inside my wallet.

3. Sometime on 15 May 2018, I placed my wallet in my bag and

went to Market! Market! in Taguig.

4. Later on in that day, I looked for my wallet, which contained

the ATM Card. However, both were no longer inside the bag.

5. I exerted diligent efforts to locate the ATM Card. However,

efforts to do so proved futile, as it could no longer be found.

6. I surmised that the wallet and the ATM Card must have been
snatched away while I was at Market! Market!.

7. I am executing this Affidavit to attest to the truth of the

foregoing and in support of my request for a replacement ATM Card.
