    Workshop on the 2024 Division Festival of Talents (DAFOT) Folkdance Competitions Contest Pieces For Elementary
    Level" on Friday January 12, 2024 at East Central School San Jacinto, Pangasinan. I have considered the benefits that
    shall be derived from the participation of my child in the said activity with the understanding that the necessary
    safety measures/ precautions shall be taken.

    However, I shall not hold the Division of Pangasinan II, San Jacinto District and staff liable if, after such safety
    measures/precautions taken, untoward incidents happen.
