AFFIDAVIT OF CLOSURE

I, AIDA A. QUIAZON, of legal age, Filipino, widow and resident of 19 Evergreen

Street, Capital Green Village, Tandang Sora, Quezon City, after having been duly sworn
to in accordance with law, hereby deposes and states, THAT:

1. I am the sole proprietor of Mica Development and Learning Center, a

church-based school for pre-schoolers and kinder;

2. On March 31, 2013, I closed the said learning center and donated it to I
am Redeemer and Master Evangelical Church;

3. The business has no financial obligation or creditor at the time of

cancellation;

4. The business was not registered in the Bureau of Internal Revenue;

5. That I am executing this affidavit to attest to the truthfulness of the

foregoing statements and for whatever legal purpose this may serve best.
