I, RAMIL DE LEON IGNACIO, of legal age, Filipino, and residents of 215 Don Jose St.,
A Hidalgo Pasaya City, after having been sworn to in accordance with law, hereby depose and say
that;

1. That I am the owner of R. Ignacio Ortho-Dental Clinic located at Baclaran, Paranaque City
and duly registered with BIR.

2. That since I operated the above-mentioned business, I personally supervised and managed
the said business;

3. That I do not have any employee/staff/worker employed on my said business, from the time
of operation until present;

4. That we are executing this affidavit to attest to the truth of the foregoing facts and
whatsoever this may serve
