POLICIES, RULES, AND REGULATIONS

GOVERNING CLASSROOM AND LABORATORY INSTRUCTION


(NOTE: with Policies on Academic Consultation and Advising Highlighted)

1. ATTENDANCE
1.1 Absences
1.11 If the number of unexcused absences incurred by the student exceed twenty (20%) percent
of the total number of hours prescribed in the curriculum, then he/she will be automatically
dropped from the subject.

2. UNIFORMS
2.1 Students are not required to wear the prescribed school uniform during online classes but are
required to wear decent attire in class and during examinations.
2.2 Gentlemen
2.21 No sporting of beard, moustache, or goatee
2.22 No female accessories like earrings and headbands
2.3 Ladies
2.31 Long hair should be neatly tied (ponytail/pigtail)

3. SCHEDULE OF ACTIVITIES
3.1 Any changes or new activities of schedules will be posted on the General Posts in Microsoft
Teams. Otherwise, these are already noted on the Course Outline given to students at the start
of the term.

4. LECTURE
4.1 Students are expected to read the assigned lecture topics before coming to class.
4.2 Students are encouraged to ask questions if they do not fully understand the lecture.

5. LABORATORY
5.1 Students are expected to read and study the assigned laboratory exercise before coming to the
laboratory.
5.2 Manuals
5.21 Drawings/results in the laboratory exercises should be submitted at the set deadline by the
discretion of the subject.
5.22 Laboratory outputs submitted late will be given deductions in the performance rubrics after
the set deadline.

6. EXAMINATIONS
6.1 All examination schedules will be stated in the course outline of the subject.
6.2 Types of Examinations:
6.21 LONG LECTURE/LECTURE QUIZ: This will include topic/s within a designated period
before the scheduled exam.
6.22 MID-TERM EXAMINATION: This will cover the first topic up to the topic which will
be announced by the examiner.
6.23 FINAL EXAMINATION: This will cover the first chapter up to the last chapter.
6.24 LONG LABORATORY EXAMINATION: The coverage will always be from the
beginning up to the exercise which will be announced by the examiner (true to MICRO
1, PARA, IMMUNO-SERO, HEMA 1, CC 1, BIOSTAT, HEALTH SCIENCE).
6.25 PRACTICAL EXAMINATION: The coverage will always be from the beginning up to
the exercise which will be announced by the examiner.
6.3 The type of examination will be at the discretion of the examiner.
6.4 Checking of the examinations will be automatically done by the Microsoft Learning Management
System. Results of the examination will be automatically generated by the system.
6.5 Students are encouraged to verify or clarify for any error in checking or computation at the time when
the results are generated. However, no complaints will be entertained two (2) weeks after the
examination results are posted.
6.6 Cheating will be dealt with severely. Any student caught cheating will be recommended
to the Dean for disciplinary action.
6.7 Special Examination
6.71 This is given only on the following conditions:
a. Illness – must present original medical certificate (scanned/photographed
copy) NOTE: Medical certificates issued by an immediate relative physician
will not be accepted. Moreover, all medical certificates should have letter
heads.
b. Death of an immediate family member – must present a photocopy of the
death certificate within 72 hours after the missed scheduled exam
c. Legal matters – must present a copy of the required document
ALL THESE DOCUMENTS MUST BE PRESENTED TO THE DEAN TOGETHER WITH
AN EXCUSE LETTER SIGNED BY THE STUDENT.

6.72 This will be given to students who were not able to take the scheduled written examination
(based on 6.71), upon presentation of an excuse slip within 72 hours which is signed by the
Dean. IT IS THE RESPONSIBILITY OF THE STUDENT TO APPROACH THE
TEACHER IN-CHARGE FOR THE MISSED EXAMINATION. The date and time of the
special examination will be scheduled by the teacher in-charge. The highest possible score
in a special exam is seventy percent (70%).
6.73 A student forfeits the special examination if he/she fails to present the excuse slip within
the specified period of time or if he/she fails to take the special examination during the
scheduled date.
6.74 In the event the student fails to take the special examination, then his/her score for a
scheduled examination will be “ten (10) points less than the lowest score” obtained in
the entire batch for that particular examination. This scoring system also holds true for
an UNEXCUSED ABSENCE in an examination.
6.75 There will be no special examination for missed laboratory practical exam. However, in
case of serious illness, communicable diseases, death of an immediate family member, or
an excuse in which the Department may deem reasonable, certain considerations may be
given to the student for the missed practical exam.
6.76 No special examination will be given to missed short lecture quiz/pop quiz.
6.77 The passing grade in all examinations is seventy (70) percent.
7. THE GRADING SYTEM:
Refer to IH Course Outline 2022
7.11 “NG” (no grade) will be given if the student fails to:
7.111 take the final written examination, or
7.112 take the final practical examination, or
7.113 submit any laboratory activity/exercises or unfinished project after the final
examination
NOTE: Upon completion of the written or practical examination, the highest possible score in
that test is seventy percent (70%).
a. For other missed examinations, the ruling in 6.7 will be followed.

2. COMPLAINTS REGARDING THE FINAL GRADE


a. Grades submitted to the Dean’s office after the Board of Promotion’s meeting will be
deemed final.
b. Request for re-computation of grades may be made through the Dean’s office.
c. Changes in the grade will only be made if there is error after re-computation and after
this has passed through the board of promotions or through the Dean’s office.

3. ACADEMIC COUNSELLING/CONSULTATION OF STUDENTS


a. A student with poor academic performance or tardiness in the class shall be called by
the faculty or instructor and be referred to the class adviser for counselling.
b. Any student with behavioural problems will be counselled by the class adviser during
his/her consultation hours. If the case cannot be settled by the class adviser, he/she will
endorse the student/s to the Dean for consultation before the case will be referred to the
Guidance office or Disciplinary Committee.

10. OTHERS
10.1 All extra electronic gadgets, i.e., cellular phones, iPads, tablets, digital/mini-cameras (e.g.,
ballpens/watches/eyeglasses), and the like are STRICTLY NOT ALLOWED during examinations.

ANY FORM OF ACADEMIC DISHONESTY WILL BE DEALT WITH SEVERELY. This will be
channelled through the Guidance Office or the Dean for disciplinary action.

10.2 All cameras must be turned on during real-time lab classes or synchronous lectures.
Playing/using game cards or mobile gaming devices during classes (both lecture and laboratory) will be
referred to the class adviser for investigation and counselling. If the case cannot be resolved
within the adviser’s level, it will be endorsed to the Dean.

11. Other rules and regulations stipulated in the Velez College students’ manual applies.

I have been given a copy of the course outline, policies, rules and regulations of IH (lec & lab)
including its grading system. I have also been oriented of its requirements and commit to passing
such requirements on time. I have had the opportunity to ask questions during the orientation
and any questions that I have asked have been answered by my instructor to my satisfaction.

I agree to the policies, rules and regulations set by the course and commit myself to adhering to
them. Should I fail to abide by the rules, I will accept whatever consequence the college deems
worthy.
