 Inferior concha:-
This is a largest of all concha and is a
separated bone.similar to the other
concha, the speac it envelops is
called the inferior meatus.
Ethmoidal Sinuses:-
These are paraied sinuses located in
between the eyes and nasal cavity.
it sinus consists of 4-18 air containing
cavities-the ethmoidal air cells.
Maxillary sinus:-
It is a largest of all sinus with a volume
of about 15 ml.These are paraied caveties
located inside the face around the area
Of the cheeks.The sinus opens into the
Middle meatus.
Frontal sinus:-
This is a last of the Sinuses develop. These are paired
Cavities Located in side the face around the area of
forehead. The Sinuses open into the middle meatus.
Sphenoid Sinuses:-
These are also paired cavities located
deep in the face behind the nose.each
Sinus has a volume of about 7.5ml. They
open into a small recess behind the
Superior meatus.
Nasolacrimal duct:-
This is a duct conveys tears from the eyes
into the nose it opens Into the inferior
meatus. Obstruction of this duct could
lead to flooding of eyes with tears and
watering of eyes.
NORMAL NOSE ANATOMY:-
 EXTERNAL NOSE –
 The nose is most prominent structure of the face.it also plays is
important, role in breathing and smell. The nasal passage sever as
an entrance to the respiratory tract and contain the olfactory
organs of smell.
 NASAL BONE
 These are paired rectangular structures attached to the skull bone
above and nasal cartilages below.
Lateral nasal cartilages
 There are two pairs of nasal cartilages, the upper and lower lateral
cartilage.
 The nasal bones and lateral nasal cartilage together from the
external nasal pyramid or vault.

 Nasal septum
This is a ventrical wall insaid the nose that
consists of cartilage In the front in the bone
in the back.The nasal septum divides the
Interior of the nose Into two principle
nasal cavitIes.

INTRODUCTION
 Membranes lining of the nose.
 It is clinically defined as a Allergic rhinitis is a
non infective immflamatory condition of the
nose and nasal sinuses.
 The symptoms usually start after an exposure to
triggering factor called an allergic rhinitis.
 Allergic rhinitis symptoms can be intermittent or
persistent and can be quite brothersome to
one’s quality of life if untreated.
 Allergic rhinitis is symptomatic disorder of the
nose indusced after allergen exposure due to an
IgE- Mediated inflammation of symptomatic
condition with four major
 symptoms as a anterior or posterior
rhinorrhoea, sneezing ,nasal itchtin and nasal
congestion.
 Triggers of allergic rhinitis
mites,domestic ,animals, insect or of plants
origin; common outdoor allergens include
pollens and moulds.
Defination Of Allergic Rhintis
 Allergic rhinitis is a inflammatory condition of
nose and nasal sinuses That Occurs When a
Person Is exsposed To something They are
allergic to. Allergens are substances That cause
allergic Reaction is Some people But cause No
symptoms In others who are Not allergic.
 Allergens can be Found indoors and
outdoors.When a Person is allergic To outdoor
allergens It is often Refferred to as “hay
fever”and occurs Seasonally.
Aetiology
 Predispositing cause:-
 Genetic factors:-It plays in important role,chances of
children developing allergy are 20% and 47%
respectively,if one or both Parents are allergic.
 Age:-It may start at infancy or at old age,but usual
age of onset 12 to 16 year of age.
 Sex :- no sex predilection
 Seasonal Variation is seen in seasonal allergic rhinitis

 Psychological factor-Prolonged stress can become a
Pathophysiology
 Sensitization to Allergens:
 Antigen-presenting cells [APCs], present in the
mucosal surface, process allergens and has some
peptides from allergens on the major
histocompatibility complex [MHC] class 2 molecules.
This MHC class 2 molecules and antigen complex
take a role as the ligand of T-cell receptors on Naive
CD4 T – cells, which results in differentiation of Naive
CD4T cells to allergen-specific Th2 cells. Activated
Th2 cells secrete several cytokines, which induces
specific IgE and proliferation of eosinophils, mast
cells, and neutrophils. Produced antigen-specific IgE
binds to high-affinity IgE receptors on mast cells or
basophils.2
 Early and Late Reactions:
 When allergic rhinitis patients are exposed to
allergens, allergic reactions develop in 2 different
patterns according to the time sequence.
 Early reaction, sneezing and rhinorrhoea develop in
30 minutes and disappears as It is the response of,
type1 hypersensitivity. The other is the late reaction,
which shows nasal obstruction approximately 6
hours after the exposure to allergens and subsides
slowly. Eosinophil chemotaxis is the main
mechanism in the late reaction, which is caused by
chemical mediators produced in the early reaction.
Several inflammatory cells, eosinophils, mast cells,
and T cells migrate to the nasal mucosa, break up
and remodel normal nasal tissue, and these result in
nasal obstruction.7

 Neurogenic inflammation:

 When the respiratory epithelium is destroyed and


nerve endings are exposed to cytotoxic proteins
from eosinophils, sensory nerve fibers are excited by
non-specific stimuli and stimulate, the so-called
retrograde axonal reflex. This makes the sensory
nerve fibers secrete neuropeptides such as
substance P and neurokinin A, which include
contraction of smooth muscles, mucous secretion of
goblet cells, and plasma exudation from capillaries.

 Non-specific hyperresponsiveness:
 It is one of the clinical characteristics of allergic
inflammation. Due to eosinophilic infiltration and
destruction of the nasal mucosa, the mucosa
becomes hyperactive to normal stimuli and causes
nasal symptoms such as sneezing rhinorrhoea, nasal
itching, and obstruction. This is a non-immune
reaction that is not related to IgE
Symptom phases:

1] Early phase symptoms:

 Runny nose
 Frequent or repetitive sneezing
 Watery or itchy eyes
 Itching in the nose, throat, or roof of the mouth

2] Late phase symptoms:

 Congestion of nose and ears feels plugged.


 Mental change can include irritability, a slight
decrease in attention span, worsened memory, and
slower thinking.

 Pale blue edematous nasal mucosa.


 Bulky edematous turbinate’s with bluish/pinkish
tingle of the mucosa.

 HOMOEOPATHIC APPROACH
 Homoeopathic medicines have an advantage in
allergic rhinitis. It helps in managing both acute
episodes as well as to reduce the chronicity.
 Allergic Rhinitis has its definite onset, location,
sensation, modalities, and concomitants, which help
to individualize the person.
 The disharmonious functioning of the vital force is
considered a disease. It is considered a chronic
disease because it has no acute exacerbation of
symptoms at the time.
 Dr. Samuel Hahnemann in his Organon of Medicine
Aphorism 117 Explained as hypersensitivity to a
substance which is the person’s own peculiar
corporeal constitution. Manifestations of
“hypersensitivity” or “hyper-responsive” reaction
(allergy reaction) and “idiosyncrasies” are
similar.There is an acquired idiosyncrasy that is born
with the patient
 Stuart Close in his lectures ‘The Genius of
Homoeopathy’ says idiosyncrasy is a habit or quality
of an organism peculiar to the individual. It is a
peculiarity of the constitution inherited or acquired
which makes the individual morbidity; susceptibility
to some agents or influence which would not so
affect others without passing to set for the more
fully the modern scientific explanation of this
phenomenon.
 As told by Dr. Kent, ‘it is a man that is sick to restore
to health, not his body, not his tissue. This tissue
could not become sick unless something before the
body’. The external disease is the out-word
expression of the internal derangement. In any
disease, not only the organs/system but the patient
as a whole is affected which is true for rhinitis too.
The balance between the environment and the
individual is distributed.
 By idiosyncrasy, we mean a habit or quality of the
organism peculiar to the individual. It is a peculiarity
of constitution inherited or acquired; which makes
the individual morbidity susceptible to some agent
or influence which would not affect other

TYPES OF ALLERGIC RHINITIS
 Seasonal: Symptoms can occur in spring,
summer, and early fall. They are usually
caused by sensitivity to airborne mold
spores or to pollens from trees, grasses,
or weeds.
 Perennial: Symptoms occur year-round
and are generally caused by sensitivity
to dust mites, pet hair or dander,
cockroaches or mold.
HOMEOPATHIC MEDICINES

 Arsenic Album
 There is thin watery discharge from
the nose with burning sensation,
recurrent sneezing one after another
with runny nose. Burning and tearing of
eyes . There is puffiness around eyes,
stuffy nose. The person has an asthmatic
tendency with breathing difficulties, and
oppression of chest which is generally
worse when he lies down. On sitting
with a forward bending posture makes
the patient little comfort from his
asthmatic problem. There is marked
wheezing sound during asthmatic
affection. The asthma is worse in wet
cold weather. Besides that the
constitutional symptoms of arsenic alb
are anxiety, restlessness, prostration,
burning sensation. The anxiety that is
found in Arsenic alb is intermingled with
fear. The other prominent symptoms are
debility, exhaustion, restlessness, with
nightly aggravation. Great exhaustion
after a slightest exertion.
 Modalities:
 Aggravation- wet
wether,aftermidnight;from cold, cold
drinks, or food.
 Amelioration- heat;from head
elevated;warm drink.
 Sulphur:
There is itching with redness of eyes in
allergic condition. The itching is followed
by burning sensation and relieved by
cold application. sulphur patient is
always irritable, depressed, thin and
weak, but good vappetite. He is having
forgetful nature, difficulties in thinking.
He has many good ideas but cannot
implement it. Selfish type person, no
regards for others. very lazy person, not
cleanness, never look after about his
dressing and clothing. Don’t like to take
bath. Having of skin diseases always. The
skin rashes itch a lot generally in night
and warm atmosphere. The person has a
feeling congestion or oppression of
chest in asthmatic condition.
 Modalities:
 Aggravation-rest, when standing,
warmth in bed, night.
 Amelioration- dry, warm weather, on
right side.
 Natrum mur:
Natrum mur is generally well indicated
in case of allergic condition which gives
the symptoms of more itching of nose,
throat, ear with recurrent sneezing one
after another. The characteristic
discharge from the mucous membranes
is watery or thick whitish, like the white
of an egg. Natrum mur patient is very
sensitive in nature. Every thing he/she
takes into the heart. The person is
irritable in nature and when in upset
does not like any body’s present or
giving of any consolation. Consolation
aggravated the state of the mind - the
melancholy, the tearfulness, sometimes
brings on anger. The Natrum mur
patient is extremely emotional. The
whole nervous economy is in a state of
fret (visible anxious like scratching finger
on her skin in worry). Completely or
desperately in love brings on complaints.
The natrum mur patient desire to take
extra salt in his diet.
 MODALITIES:
 Aggravation-heat, warm room, talking,
music.
 Amelioration-open air, cold bathing,
tight clothing.
 Sabadilla:
it has good action on mucous membrane
of the nose and the lachrymal glands,
producing coryza and symptoms like
hay-fever. There is spasmodic sneezing
one after another. symptoms of hay-
fever or allergic rhinitis with itchy nose
and fluent coryza. Either nostril stuffed
up, inspiration through nose labored,
snoring. Violent sneezing is occurred
from time to time, shaking abdomen
followed by lachrymation. There is runny
nose with severe frontal headache and
redness of eyelids.
 Modalities:
 Aggravation-cold and cold drinks, full
moon.
 Amelioration-warm food and drink,
wrapped up.
 Allium cepa:
Allium Cepa is one of the most
commonly used Homeopathic medicines
for Allergic Rhinitis or Hay Fever. It is
generally used in the symptoms of
severe runny nose that drips from the
tip of nose with watery eyes. There is
burning of nose due to over secretion
mucous. Along with the watery nasal
discharge there is watery eye. There is
profuse sneezing associated with runny
nose and watery eyes.
 Modalities:
 Aggravation-in the evening, in warm
room.
 Amelioration-in open air, and in cold
room.
 Tuberculinum:-
-The patient is very susceptible to catch
cold. The physical constitution is lean and
thin like natrum mur, but not obese like
calcarea carb. If there is family history of
tuberculosis or bronchial asthma then is
more suitable to give this medicines in any
allergic or asthma condition. The person is
very depressed, melancholic, taciturn
sulky nature.
Modalities:
 Aggravation-motion, music; standing;
dampness; early morning, and after
sleep.
 Amelioration-open air.
 Arundo mauritanica:
 Arundo mauritanica: -
-When the episode begins with intense
itching in the Palate and the nose,
arundo is very much suited to the case
and is one of the best homeopathic
medicine for allergic rhinitis or hay fever.
-There is a lot of burning and itching in
the Nose the eyes and the roof of the
mouth. Sense of smell diminished.
 Merc sol:-
- One of the best homeopathic
medicines for allergic rhinitis with
sensitivity to heat and cold when there
is increased sensitivity to both heat
and cold and the patient cannot
tolerate either, merc sol is very
effective and is one of the best
homeopathic medicine for allergic
rhinitis or hay fever.
- There is a lot of sneezing and watery
discharge from the nose.
- The patient is worse even in damp or
wet weather.
- Increased sweating is another
indication of this medicine.
- There is profuse salivation in the
mouth.
- At time the salivation is so much that it
keep the mouth wet most of the time.
- Despite this increased salivation, the
patient keeps on feeling thirsty.
- There may be foul smell from the
mouth.
 Modalities:-
 Aggravation-night, damp cold, heat
 Amelioration- rest,coition
CASE SUMMARY
Case 1
 A 22 yr old Male Pt his complaint of
Cold,sneezing,lachrymantion and watery
nasal discharge since Last 4 months.The
pt was under allopathic treatment
earlier which gave him temporary
relief.The Sneezing started Suddenly 15-
20 in row,and in aggravated in
morning,dust mites,Cold drink,change of
weather.warm drink relives for
Sometime.During this,he had profuse
watery nasal discharge with itching of
nose and Profuse lachrymantion With
burning and redness of eyes.
 past history:-Nothing specific not found.
 Family History:-Nothing contributory
History found
 PHYSICAL GENERAL:-He has a good
appetite and desire for sweet. He
become thirsty so often and drinks 3 lit
of water has regular bowels movement
and normal micturition. Tongue clean
and moist and skin is healthy. Sleep
disturbed as nose get blocked at night.
 Thermal :-chilly
 Mental symptoms:-The patient said only
thing that was bothering him is not
getting relief in spite of taking
medicine.he gets irritated easily since
the complaints started become angery
on seeing mess in the room , wants
everything neat and clean.
 On systematic and general examination:-
on auscultation, wheezing sound heard
our both side of lungs. Body
temperature was 98 F. blood pressure
and pulse rate were 130/80 mm/hg and
82beats/mint respectively.
 Local examination:-swollen nasal
turbinate. Redness and congestion of
nose and throat. Redness of eyes.
 Analysis of case:-after case taking all
mental, physical generals particular

-Among mental symptoms:-anxiety,
irritable fastidious and physical general
unquenchable thirst, disturbed sleep
were taken for constructing the totality
among particulars, sneezing, cold
aggravation, morning aggravation, dust

- Repertorization was done by using

 A 25 years old male patient His complaint of stuffed
stoppage of nose with watery discharge and
frequent Sneezing. This complete get aggravted in
evening and in dry, cold weather.profuse sweating
all over the body with odour and stain.tendancy of
getting cold easily.he has become irritable and
restless due to present complaint.
Present complaint:- location- upper respiratory tract-
