I, Francisco C. Enar, of legal age, Filipino, married and with residence

and postal address at Brgy. Kalamigan, Concepcion, Iloilo after having been
duly sworn to in accordance with law, depose and state:

1.) That I am the Petitioner in the foregoing Petition; that I have

caused the preparation of the said Petition , that I have read and understood
all the allegations contained herein and that they are all true according to my
personal knowledge, available documents and authentic records;

2.) I further certify that I have not commence any other action or
proceeding involving the same issues in the Supreme Court, Court of Appeals,
or different Division thereof, or any other tribunal or agency; and that to the
best of my knowledge, no such action or proceeding is pending before in the
Supreme Court, Court of Appeals or different Divisions thereof, or any other
tribunal or agency, and that if I should thereafter learn that a similar action or
proceedings has been filed or is pending before the Courts or Tribunal, I shall
