The Barangay Council of Barangay Sto. Nino, City of Koronadal, South Cotabato would like to
request Three (3) Evaluators from the City Environment and Natural Resources Office
(CENRO) – Ms. Marietta Dasilao, City Health Office (CHO) – Ms. Mylene Seliote and City
Agriculture Office – Ms. Arlene Binto for the 3rd Quarterly Evaluation 2023 - assessment of our
Clean and Green Contest Program of Barangay Sto. Niño, City of Koronadal, South Cotabato.

Anent to the foregoing, we will also be requesting the above-stated persons to be available on
November 20, 2023 at around 8:00 AM onwards for the assessment of our Seventeen (17)
Puroks. We plan to implement and conduct the abovementioned activity in compliance with
regulatory standards and to keep abreast with the schedules of our annual program.

Your support will be highly appreciated. We would like to extend our utmost and sincere
gratitude for your generous assistance and we shall commit to comply with all the requisites
needed for the approval of our appeal.
