    I voluntarily surrendered my ATM to expedite the loan obligation re-payment with the
    Company. I humbly request Millennium Cash Finance Company, Inc. – Sta. Cruz,
    Laguna Branch to personally collect money from my ATM Card, since it will be
    convenient for me to collect the remaining balance from my monthly amortization.
