    Date: January 23, 2023

    Herewith is the Regional Memorandum dated January 18, 2023 from the office of the
    Regional Director, ASD-2022-011 entitled, "Uniform Template for Deped Seroice Recor<f'.

    Effective February 15, 2023, this Office shall only receive the Service Record
    template attached herewith that shall be released by the Schools Division Office and
    Implementing Units for Retirement, Survivorship, Terminal Leave, other related claims or
    requests and whatever legal purpose it may serve the requesting party.
