Vaginal leiomyoma

INTRODUCTION: Vaginal tumors are infrequent occurrences. They include haemangioma,

papilloma, mucosal polyp, and leiomyoma. Leiomyomas can affect 20–30% of women of

reproductive age and are commonly observed in the uterus. They can also occur in a lesser range in

the cervix, uterosacral ligament, round ligament, ovaries, and inguinal canal. However, affecting the

vagina is exceedingly rare, representing the least common presentation among all locations.

CASE PRESENTATION: A 50 years old lady presented with mass per vagina since 9 years with

insidious onset, initially it was peanut size now progressed to pebble size with no other complaints.

Physical examination revealed a round, smooth mass of size 6*8 cm in the right lateral vaginal wall

mucosa over mass normal, with normal looking cervix, in the mid position. MRI showed polypoidal

lobulated mass measuring 8.1*5.5*4.3 cm arising from right lateral vaginal wall extend posteriorly

involving external anal sphincter from 7to 12’clock position and levator ani muscle on right side. she

underwent enucleation of mass through vaginal route, no excessive bleeding, bed sutured with

intermittent sutures. Postoperative period uneventful. HPR of the report came as leiomyoma.

CONCLUSION: Vaginal leiomyoma is a rare tumor with approximately 300 reported cases worldwide.

It can be asymptomatic or cause to feel it within the vagina. Besides the necessity of surgical

resection, colporrhaphy as a reconstructive surgery may be required.
