iFR Scout Pullback Measurements ™

Step 1: iFR® Spot Measurement

„„ NORMALIZE pressure wire and advance distal to all lesions in the vessel iFR Zone FFR Zone iFR Zone
„„ Flush contrast out of the guide catheter and wait for Pa to stabilize

Step 2: iFR Scout™ Pullback

„„ Tighten hemostasis valve to avoid blood loss but so that
pressure guide wire can still be pulled back
Press PULLBACK

After the blue lines appear, wait until the blue line stabilizes

and the iFR® value displays a similar result to the “spot”

Slowly pullback the pressure wire at a constant speed
„„

(20-30 sec) while observing the sensor position under fluoro

(sensor is located just proximal to the radio-opaque tip)
„„ Press BOOKMARK on the console to flag areas of interest,
such as lesions, during the pullback recording
„„ Press STOP when complete

Step 3: Review the Results

„„ If the pullback was performed
at relatively constant speed:
»»Steeper slopes may 3
indicate focal disease 3

»»More gradual slopes
may indicate diffuse disease 2 1

0.85 to 0.92.

Step 5: Compare the “Pre-Pullback” to the “Post-Pullback”

„„ Observe your FUNCTIONAL GAIN by making a final pullback
»» Reposition the radio-opaque sensor at the same initial position
distally in the vessel from the previous measurement

»» After the Post-pullback recording, ensure that the iFR® scale

is identical to the previous pre-pullback recording. If necessary,
adapt the iFR® scale in “OPTION” menu and “iFR® Pullback Range”

1. An iFR cut-point of 0.89 matches best with an FFR ischemic cut-point of 0.80 with a specificity of 87.8%
and sensitivity of 73.0%. (iFR Operator’s Manual 505-0101.23).
