COMPLAINT AFFIDAVIT

I, Kristina Ildefonso of legal age, single, and with postal address at

123 Maganda Street Santa Ana, Manila City after having been duly sworn to
in accordance with law do hereby depose and say;

1. I am instituting this complaint affidavit against James

Malapote w h o i s o f l e g a l a g e , s i n g l e , F i l i p i n o c i t i
z e n a n d resident of 987 Chaka Street, Santa Ana, Manila City.

2. Respondent was my ex-boyfriend, our relationship lasted for 5

years, from January 1, 2019 to January 2, 2024.

3. By October 10, 2023, his brother was hospitalized due to accident

and he needed money for hospital bills as well as medications.

4. By reason of love and trust, I lend him a money last October 15,
2023 amounting to P20,000 (twenty-thousand pesos).

5. By November 2, 2023, I found out that James Malapote cheated

on me with another girl named Kristel Alvarez.

6. Thus, I asked James Malapote to pay the P20,000 he borrowed,

and I gave him 30 days to pay.

7. Since then and for numerous times after the 30-day period, I
demanded him to pay the total amount, however there was no
response at all.

8. On January 4, 2024 the respondent even once replied to my

message through messenger stated “Hindi ko na babayaran yan,
bahala ka na jan”.

9. I am executing this Complaint-Affidavit to attest to the facts stated

above and to substantiate this complaint against the respondemt.
 That I attest that the foregoing statements and the submitted
supporting documents are true and correct to the best of my knowledge and
belief and I commit myself to be liable for perjury and/or dishonesty as a
result of any fraud, false or misrepresentation in this affidavit.
