I am writing to refer Mrs. Johns, a 52-year-old smoker, who needs root canal treatment
of her lower left second molar. Her root canal treatment was performed one month ago
that was difficult due to obstruction in the mesio buccal canal.
Mrs. Johns was diagnosed with myocardial infarction one year ago, for which she takes Commented [DR1]: Is taking
aspirin 100mg daily. She is also allergic to penicillin.

An initial examination revealed that Mrs. Johns is a regular attender and has missing Commented [DR2]: was
some of her teeth. She has had RCT’s and amalgam fillings in her teeth. She was
presented with complaints of pain and abscess in the lower left second molar. After an
examination, root canal treatment was planned. During root canal treatment mesio

canal was unable to instrument beyond 8mm. A periapical radiograph was performed, Commented [DR3]: could not be instrumented beyond
