I am writing to refer Ms. Rosie Buck, a 47-year-old smoker, who presented with a complaint of post
crown pain around the tooth 11 caused by a deep pocket around the mesial surface of the tooth Commented [A1]: post-crown

anaesthesia and penicillin.
On 01 July 1995. an initial examination revealed that Ms. Buck has had an amalgam and composite Commented [A5]: ,
fillings along with carious lesions in the teeth 16, 26, 24, 43 and 11. Moreover, she has sub-gingival Commented [A6]: ,

On 28 July 1995, root canal treatment on tooth 11 was started and erythromycin 250mg was

prescribed. Root canal treatment was completed after 2 weeks and a post crown was inserted after
2 months along with fine scaling.
0n 05 February 1997, Ms. Buck presented with pain on and around post crown on the tooth 11. X- Commented [A10]: of
ray indicates a deep pocket on the mesial surface of the tooth 11. This tooth was sensitive to Commented [A11]: indicated
pressure.
