AFFIDAVIT OF LOSS

I, ARJOHN P. ESPINO, of legal age and residing/with address at

Las Piñas City, NCR, after having been duly sworn to in accordance with
law, depose and state THAT:

1. I am the same and identical person who executed this Affidavit of loss.

2. I am a holder of Hyundai Maintenance Service Passport issued by HYUNDAI

3. Sometime on January 31, 2023, at around 01:30 PM Drove to Shell C-5 Las
Pinas for a carwash service. brought my documents in a folder containing my
Company car OR/CR and service maintenance passport which I usually store
in the glove compartment. Left it at the lounge and recalled only 30 mins.
after. When I returned to the lounge, folder was not in the table anymore.
Asked the service crew if anyone had picked up the folder but to no avail.

4. Despite diligent efforts to retrieve or otherwise recover Hyundai

Maintenance Service Passport the same could no longer be recovered.

5. For purposes of applying for reissuance of the of Hyundai Maintenance

Service Passport Issued By, HYUNDAI; I am compelled to execute this
affidavit of loss;

6. Thus, I am executing this affidavit of loss to attest to the truthfulness of the

foregoing, to request for replacement, and for whatever legal intents and
purposes it may serve.
