I, ARIES B. PAGKALINAWAN, Filipino, of legal age, single, and with of-

fice address at 6th Floor The Currency Bldg., Julia Vargas Ave., Ortigas,
Pasig City, after having been duly sworn in accordance with law, hereby depose
and say:

1. That I am the Liaison Officer of South Asialink Finance Corp. ("SAFC" for
brevity)

2. That as part of my job description, I am the custodian of the OR/CR of all the
SAFC clients that needs to be encumbered and annotated upon their loan
approval;

3. That I was able to get hold of the OR/CR of Ms. Floresita Cervantes,
one of the clients/borrowers of SAFC from Head Office Branch;

4. I received the copy of the original OR/CR sometime October 2021 for process-
ing of encumbrance;

5. That on 25 October 2021, at around 9:30 in the morning when I reached the of-
fice, and preparing for some fieldwork, I am supposed to bring with me
the OR/CR of Mr Del Rosario for annotation but I could no locate the
same;

6. I tried to look for it within the vicinity of our office and even went outside and
to the parking area but found to be negative;

7. That diligent efforts to locate the same had proved futile;

8. That I am executing this affidavit to attest to the truth of the foregoing facts
and for any legal purpose it may serve.
