PRE-EMPLOYMENT CHECKLIST

1. The list of pre-employment requirement contained in this document should be monitored by HR Assistant assigned at On Boarding.
2. To ensure 100% submission and compliance, incomplete requirements will not be accepted unless there's a valid reason.

DURING THE JOB Assigned Recruiters provide the following pre-employment documents during the Job Offer:
Recruiter a) Job Offer and Employment Contract
b) Pre-Employment forms
New Hires accomplish the following pre-employment forms on the same day as the Job Offer:
(Strictly before the NEIP date)
a) Resume (signed each page, 2 copies)
b) Transcript of Records (2 Photocopies)
c) Authorization for Background Investigation
d) YONDU ID Application Form
e) Globe Security Pass Application Form (for globe based)
f) Group Life Insurance Form
g) PMRF (Philhealth Member Registration Form)
h) Residence Sketch (2 copies)
HR Assistant HR submits the signed Resume, TOR, Sketch of Residence and Authorization for Background Investigation documents to the accredited
vendor.
2 WEEKS PRIOR Recruiter New hires must be advised and required to submit the following requirements:

a) 2 pcs of 2R size photo (Half Body Photo in white Background)
b) 4 pcs 1x1 ID Picture (white background)
c) 4 pcs 2x2 ID Picture (white back ground – for Globe Deployed)
d) Barangay Clearance (Original)
e) NBI Clearance (Original)
f) Police Clearance (Original)
g) Photocopy of any (2) two Government Issued ID (to be validated)
h) Birth Certificate (PSA/NSO Photocopy) (4pcs)
i) Birth Certificate of spouse (PSA/NSO Photocopy) (4pcs) (if applicable)
j) Birth Certificate of child/children (PSA/NSO Photocopy) (4pcs) (if applicable)
k) Marriage Certificate (Photocopy) (if applicable)
l) SSS E1/E4/Employee Static Information Form (Photocopy)
m) PhilHealth ID/MDR (Photocopy)
n) Pag-IBIG MDF (Photocopy)
o) 1905 (Must be with receive stamp of RDO/Registry District Office to residence)
p) Diploma (Photocopy)

DURING THE ON HR Assistant 1. HR validates submission of pre-employment requirements. In cases of incomplete
BOARDING DAY submission, new hires are given 1 week to fulfill pending requirements .
3. Background Check Results Validation and Review.
4. All Client-based new hires are endorsed to Resource Managers for briefing sessions
after completion of On Boarding General Courses.
5. Employees are required to proceed to designated BPI Servicing Branch to facilitate for
their payroll account.
WITHIN 1ST WEEK HRBP / RM Immediate Superior provides Job Description and Key Results Areas. These should be explained and signed by both parties (IS and
AFTER employee).
ONBOARDING
30-60 DAYS AFTER HR Assistant If available, the following documents should be submitted during the on boarding date. If not yet available, new hires should be instructed
ONBOARDING to submit within 30 – 60 days from the date of hiring.
a) COE from previous employer (Photocopy)
b) Clearance from previous employer (Photocopy)
c) 2316 from previous employer for the current year (Photocopy)
COMPLIANCE HR Assistant 1. HR validates that all pre-employment requirements are submitted by the new hires.
CHECKPOINTS
2. In the event that there are still pending requirements, new hires are given 30 to 60
days to complete.
3. HR conducts an audit to ensure 100% submission and completion of pre-employment
checklist every 4th week of the month.
4. Sales Admin Staff shall validate submission compliance for client-based employees,
while HR shall validate compliance submission for all new hires for HQ on a monthly
basis.
5. HR maintains New Hire Tracker and ensures that the list is updated on a monthly basis.

This is to certify that all pre-employment requirements are submitted by (Name of Employee) and the same was validated by the
undersigned in collaboration with Talent Acquisition and Talent Solutions Group.
