Preface
Introduction
PHE ONWJ Lifting Management System (PHEONWJ-G-PRC-0007) is applicable in all areas of the
PHE ONWJ organization.
The online document is to be considered the master version, containing the most up to date
information.
Review and Update
This document will be subject to 48-monthly review and update, when document holders will have
the opportunity to express opinions and suggest improvements.
However, the document control system allows for continuous update of this document. As such,
any user may at any time identify an error or suggest an improvement.
All holders of this document are registered so that they can be sent updates and be kept informed
of changes or reviews.
Responsibility of Copyholders
It is the responsibility of the registered copyholder of controlled hard copy versions to maintain the
accuracy of the document by ensuring that all updates are promptly incorporated and
acknowledged.
Furthermore, the registered copyholders of controlled hard copy versions must at all times maintain

1. General
1.1. Background
The increasing yearly production target of PHE gives direct impact on amount and complexity of
lifting operation. The activity of lifting operation comprise of planning, selection, usage, handling,
maintenance and repairing of lifting device. Every department whom related to this operation has
its own program and work plan. This activity needs supervision and control during implementation.
As a prevention of workplace accident each department needs a supervisor to ensure that lifting
activity conducted secure, effective, and efficient. The ideal lifting activity could be obtained by
following these procedures:
- Ensure that selection, usage, maintenance, inspection, and repairing of lifting device are
well conducted.
- Ensure that the selection, placement, and assignment of workers are conducted correctly.
- Ensure the availability of lifting procedure according to location and ensure implementation
of procedure
A management system is needed in order to manage items related to lifting device, lifting assistive
device and lifting activity. This system also will ensure:

- The selection, utilization, maintenance, inspection, and repairing of lifting and lifting
assistive device will be done in accordance with manuals or applied international standard.
- All personnel from higher level supervisor to activity implementer are available and
knowledgeable about their job and responsibilities.
- The equipments that will be lifted are prepared, equipped with lifting device and ensure on
good condition
- The monitoring of work performance by work system. The system will allow the higher
authority to give suggestion to ensure sustainable improvement.

1.2. Scope
This document covers lifting management personnel includes lifting technical expert, onshore
responsible person, offshore responsible person, and lifting engineers. The explanation of lifting
devices will cover procurement, maintenance, repairing and certification of lifting devices.

2. Lifting Management
Lifting management explain positions whom responsible during lifting activity. These positions have
to be ascertained that all procedure, equipments and lifting personnel are comply to the work
necessity , government regulations and applied code and standard.

2.1. Gatekeeper
Gatekeeper will be responsible for
- Set the expectations and ensure alignment with business strategy
- Approve the Key Performance Indicators
- Provide guidance and assistance to the lifting technical expert in securing resources and
funds
- Approve lifting management Organization
- Review and approve the lifting management system
- Review the LMS implementation progress with lifting technical expert.

Gatekeeper role will attach to Senior HSSE Manager and Lifting technical expert must update the
lifting management system implementation progress in monthly basis to Gatekeeper.
2.2. Lifting Technical Expert
Lifting Technical Expert will be responsible for
- Creating regulations to cover general lifting activity on PHE ONWJ includes offshore and
onshore activities
- Ensure the procedure has been informed and well conducted
- Ensure every platform, project, barge, ships, etc has applicable lifting procedure
- Information source to Onshore Responsible Person, Offshore Responsible Person, OIM,
Man In charge if there’s some d oubt on selection, maintenance and repairing of lifting

device.
- Review and give approval to OIM if some deviation occurs during operation, maintenance
and repairing of lifting device.
- Approve specification of lifting devices
- Approve qualification lifting personnel
- Perform audit and review during for lifting equipment selection, operation, maintenance,
and repair.
This position will be held by crane & lifting maintenance team leader and will be positioned on PHE
ONWJ office.

2.3. Lifting Engineer


This position is sub ordinate from lifting technical expert.
The responsibilities of lifting engineer will cover but not limited to:
- Ensure all location own ‘Data base of lifting d evice and personnel’ in accord ance with

requirement.
- Maintaining the data base of lifting personnel who works on PHE ONWJ area
- Maintaining the data base of PHE ONWJ lifting devices which includes but not limited to
cranes, forklift, lifting gears, cargo carrying units, and personnel transfer capsules
- Recommend lifting device specification includes the prediction of device amount during
selection phase.
- Recommend qualification of lifting personnel during selection phase.
- Give recommendation on selection of lifting device
- Give recommendation if some deviation occurs on lifting activity
This position will be held by operation technical maintenance crane & lifting maintenance
supervisor and will be positioned on PHE ONWJ Office

2.4. Onshore Responsible Person


This position will be responsible as supervisor of activity implementation on each department and
ensuring that all activity performs securely and comply with applied code and standard.
The responsibilities of Onshore Responsible Person will cover but not limited to:

- Ensure all company procedures are well informed and well implemented on every
department
- Ensure specific lifting procedures that will be used on every platform, project, barge, ship,
etc. are complying with designated area. The procedure will also have to comply or has
higher specification than PHEONWJ-SOP-U-100
- Ensure specification and amount of lifting device needed on every department operation
area.
- Ensure qualifications and amount of lifting personnel needed on every department
operation area

- Ensure draft lifting plan and L2RA for lifting activity planned and prepared from onshore are
available and given to Offshore Responsible Person.
- Ensure that all deviation during operation activity, maintenance, and repairing of lifting
devices available if needed.
- Ensure all implementation of operation activity, maintenance, and repairing of lifting
devices are comply with manuals and applied regulation.
- Ensure that auditing and reviewing of selection, operation, maintenance, and repairing of
lifting devices are conducted on every department.
This position will be available on every department and decided by each department senior
manager from recommendation of related manager.

2.5. Offshore Responsible Person


This position will be responsible that lifting activity in each facility will be conducted securely and
comply with regulation and applied code and standard.
The responsibilities of Offshore Responsible Person will cover but not limited to:

- Produce specific lifting procedure that will be used on every platform, project, barge, ship,
etc are comply with designated area. The procedure will also have to comply or has higher
specification than PHEONWJ-SOP-U-100.
- Maintain specification data base and amount of needed lifting device on every facility.
- Maintain qualification data base and amount of needed lifting personnel on every facility.
- Produce and preparing lifting plan and LR2A to be approved by OIM/Man In charge
- Maintain data base of lifting plan and LR2A on specific area.
- Ensure all activity of operation, maintenance and repairing of lifting device are well
conducted according to manual book and applied regulations.
- Conduct auditing and review on operating, maintenance, and repairing of lifting device on
each facility.
This position will be available on every facility and decided by each department manager from
recommendation of OIM/Man in charge.

2.6. OIM/Man in Charge


The position is structural function and apart directly from lifting organization but as a holder of the
highest authority on installation, the involvement on lifting activity is really crucial.
Duties and Responsibilities:

- Ensure that all regulation according to statute on the operation area is fulfilled
- Give final approval on lifting plan
- Suggest Offshore responsible person on each operation area

2.7. PSCM or Contract Engineer


This position is not directly related to daily lifting activity, but it is really crucial on ensuring
procurement of lifting device through tender is well conducted.
Responsibilities:

- Ensure all requirement on lifting device and personnel are attached on scope of work on
contract document
- Ensure that contractor capabilities on providing the lifting devices and lifting personnel are
considered as factor on contractor selection

- Ensure that all company regulation about lifting activity implementation are communicated
to supplier and contractor
- Ensure all suppliers and contractors are informed compliance on lifting activities will part of
contractor performance assessment.

2.8. Contract Holder


This position is directly separated from lifting organization. Since this position will act as owner or
contract supervisor therefore the duties and responsibilities is really vital on supporting safe and
effective lifting operation.
Responsibilities:

- Ensure all the requirement of lifting device and personnel are attached on scope of work
on contract document
- Ensure that contractor capabilities on supplying lifting device and personnel used as factor
on contractor selection
- Ensure all company regulations about lifting operation are socialized and implemented by
suppliers and contractors
- Ensure all suppliers and Contractor Company prepared all material to be lifted on good
condition complete with proper lifting devices.
- Ensure the competency and quantity of lifting personnel
- Ensure the specification, quantity, and condition of lifting device are meet to requirement
- Ensure the implementation of inspection, maintenance, and repairing of lifting devices are
well conducted
- Conduct audit and review on contractor performance for at least once every 3 months.

2.9. Responsible Accountable Consult Inform Analysis


During the implementation of their duties, lifting management has connection one to other.
The following table shows the connection during implementation.
Definition:
R (Responsible) refer to the party whom assigned to implement the activity
A (Accountable) refer to the party who took decision and main ownership
C (Consult) refer to the party that could be asked for the opinions before decision making
I (Inform) refer to the party that has to be informed before decision making

3. Lifting Equipment Management


3.1. Lifting Equipment Selection
Every operated lifting device on PHE ONWJ operation area must have identity, valid certificate and
only use according to its capacity.
Therefore on every facility and project the OIM/Man In Charge must identify the requirement of
lifting device. This requirement must be informed to the Onshore Responsible Person so the
requirement list and its specification could be prepared. This list then verified especially for the
amount and the type of lifting device whom unavailable to be further purchase or rent. The
purchasing and renting of required devices must involve PSCM. The detail of the type and amount
of require lifting device must be included on bid document.

3.2. Lifting Equipment Register


The lifting eq uipment’s and lift gears belong to PHE ONWJ must own identity. The specification of
the identity will be as following.
- Crane : Location (BPRO,BCS,B1C) - Cr
- Overhead Crane : Location ( BPro,BCS,B1C)- Ov-Cr
- Forklift : Brand Type Number ( 01,02,03 etc)

- Personnel Transfer : Brand Type- Number ( 01,02,03 etc)


- Rigging Loft : RBM-Number (01,02,02,03 etc)


- Cargo Carrying Unit : PHEONWJ-CCU-Number (01,02,03,etc)

The lifting and lifting gears that own by contractor will be manage by procedure or
eq uipment’s

management system of the contractor.

3.3. Lifting Equipment Operation and IMR


3.3.1. Lifting Equipment Operation
The lifting equipment and lift device has different capacity and function. The usage of lifting
devices should maintain below each capacity. The application must be in accordance with
operation manual or related code and standard.

3.3.2. Lifting Equipment IMR


To ensure lifting devices integrity as per design, it is required to perform regular inspection,
maintenance, and repairing in accordance with the devices manual book and the state
regulation or in accordance with applied code and standard and state regulation

3.3.3. Lifting Business Process map


The lifting business process map is created to explain the implementation flowchart from
selection, operation, maintenance, repairing of devices and auditing on procedure fidelity.
This map is shown on figure 2

4. Lifting Personnel
Lifting personnel who has direct involvement on lifting operation gas to be identified, selected, and
appointed according to its competency. The qualification and competency of lifting activity covers
but not limited to Crane Operator, Forklift Operator, Signalman, Lifting Rigger, Maintenance
Supervisor, Lead Maintenance, Mechanic, Electrician, Roustabout. The diagram of authorities on
lifting operation is shown on figure 2

The qualification of lifting personnel could be varies depend on the requirement of activity. The following table

If during implementation some there are difference between procedures, code, and government
regulation. Then it must follow the priority of:
1. Government regulation
2. International Standard Codes
3. Company procedure
4. Best practice or guidance and manual book.

6. Load
The usage of cargo carting unit is the best solution to lift material or load. The material or load will
have to be clarified its weight, content, lifting point before being lifted, removed or installed.
If the load can be lifted directly without using cargo carrying unit it must be ensured that the
condition of cordage point and cordage methods are secured and unharmed to the lifted material.
Inspection and maintenance to lifting point must be done periodically.

7. Lifting Audit
To ensure fidelity on lifting management system during implementation, auditing will be done
periodically.

7.1. Monthly Auditing


The auditing for field operation will be done once every month on each area. This audit covers
fidelity audit during lifting activity, and ensuring all activity comply with the procedure. The specific
audit will cover lifting operation activity, inspection work, maintenance and repairing the lifting
devices.

7.2. Quarterly Auditing


General audit of Compliance to LMS implementation will be done once every 3 months. Lifting
technical expert will create auditing schedule on every department. The monthly audit report on
each site will be used as base line information of field audit. Meanwhile, general audit on all lifting
management system elements will be done based on data base from onshore responsible person.

7.3. Performance Review


As performance tracking, audit report will be informed to contract holder and PSCM and will be use as input on Performance Review.
