    CHECK LIST FOR APPLICATION OF MAXIMUM RETAIL PRICE (MRP) OF


    CHANGED OR ADDITIONAL PACK OF ALREADY EXISTING DRUG(S) AS PER
    POLICY PROVISIONS OF PARA 5(3) OF DRUG PRICING POLICY-2018.

    1. Fee Challan in original as per DRAP notification.


    2. Certified true copy/attested copy (by notary public) of registration letter of the existing
    pack size.
    3. Certified true copy/attested copy (by notary public) of renewal of registration letter (if
    applicable).
    4. Specimen label/carton of existing pack(s) notarized as attested/certified true copy.
    5. Certified true copy/attested copy (by notary public) of latest price list of all registered
    products of the firm/company.
    6. Certified true copy/attested copy (by notary public) of valid Drug Manufacturing License
    (DML).
    7. Certified true copy/attested copy (by notary public) of Central Research Fund (CRF)
    certificate.
    8. An affidavit by the owner or Managing Director/CEO of the firm/company on stamp
    paper of Rs. 20/- as per specimen attached duly attested by notary public as “certified to
    be true copy”.
