SAFETY RULES (MECHANICAL)


SECTION 1
GENERAL REQUIREMENTS

1.
DUTIES

It is the duty of all agents, contractors and employees working


in generating stations or on their associated equipment to
conduct their work in accordance with these Rules.

The Company requires all persons to make full use of any means or
appliance provided for the purpose of securing safety, and also
not wilfully to do anything likely to endanger themselves or
others.

All persons who may have to work or be concerned with the


operation of the Company's generating stations or their
associated equipment, must make themselves thoroughly conversant
and comply with the relevant statutory requirements and with
these Safety Rules. Ignorance of the requirements or Rules will
not be accepted as an excuse for neglect of duty.

Notices and documents shall be posted or made available as


specified in Appendix 'A'.

2.
OBJECTIONS ON SAFETY GROUNDS

Any person receiving instructions in the application of these


Rules shall report to the person issuing those instructions any
objections on safety grounds to carrying them out. Any such
objections shall be investigated and if necessary referred to
higher authority.

3.
ISSUE OR REVISION OF SAFETY RULES

A copy of the Safety Rules shall be issued to such persons as may


be deemed to be concerned with the operation of, or work on, the
Company's system or plant. Such persons shall sign a receipt for
their copy, which shall be kept in good condition.

Further copies of the Safety Rules shall be posted or made


available at generating stations.

Revision to Safety Rules shall be reviewed by the Safety Rules


Committee and approved by the Managing Director or person
nominated by him in writing.

4.
DEFINITIONS

For the purpose of the Safety Rules the following definitions


apply.

APPARATUS - any item of electrical equipment which

transformers, switchgear, conductors and
cables.

APPROVED - of a type sanctioned for use by the


(經批准) Managing Director of the Company or
person nominated by him in writing and
subject to a current test certificate
where applicable.

AUTHORISED PERSON - a Competent Person possessing technical


(授權人士) knowledge and a valid Certificate of
Qualification to carry out specific
operations or work on Mechanical Plant.
The Certificate of Qualification may
include the authority to issue, receive
and cancel Limitation-of-Access,
Mechanical Permit-to-Work, Confined Space
Permit-to-Work and Sanction-for-Work.
In any part of the Safety Rules, an
Authorised Person refers to an
appropriate Authorised Person who
possesses the relevant Certificate of
Qualification that allows him to carry
out the duties mentioned in that part of
the Safety Rules.

BARRIER - an Approved means of physically


(欄柵) preventing unauthorised approach to a
source of Danger.

CAUTION NOTICE - a notice in Approved form attached to


(注意告示) Mechanical Plant or its control equipment
conveying a warning against interference
with such Mechanical Plant.

CAUTION FIRE - a notice in Approved form attached to

CERTIFICATE OF - a certificate issued by the Managing


QUALIFICATION Director of the Company, or person
(資格證書) nominated by him in writing, stating the
class of operation or work the recipient
is qualified to carry out and the section
of the system to which it applies.

COMPANY - The Hongkong Electric Company Limited,


(本公司) 香港電燈有限公司. The abbreviation is HEC.

COMPETENT PERSON - a person who has sufficient experience to


(合格人士) enable him to avoid Danger and who is in
possession of a valid Certificate of
Qualification. In any part of the Safety
Rules, a Competent Person refers to an
appropriate Competent Person who
possesses the relevant Certificate of
Qualification that allows him to carry
out the duties mentioned in that part of
the Safety Rules.

CONFINED SPACE - any chamber, tank, pit, well, culvert,


(密閉空間) tunnel, flue, boiler, pressure receiver
or other space which lacks ventilation,
or a place where dangerous fumes are
liable to be present to such an extent as
to involve risk of persons being overcome
thereby.

CONFINED SPACE - a form of declaration signed and issued by an Authorised


PERMIT-TO-WORK Person to a Competent Person in charge of work to be
(密閉空間工作許可證) carried out in a Confined Space. The declaration
shall make known to such person exactly
what plant is Isolated from all sources
of Danger and shall specify what testing
has been conducted and what precautions
are necessary whilst work is in progress.

CONTROL ENGINEER - an Authorised Person appointed in writing


(控制工程師) by the Company for the control and safe
operation of the transmission,
distribution or generation system as
specified in the System Operating
Procedure.

CRYOGENIC FLUIDS - gases that have been liquefied and are


(低溫流體) characterized by extremely low
temperatures and high rates of expansion
when changing from liquid to gas phase.

DANGER - a risk of bodily injury or loss of life


(危險) or health.

DANGER NOTICE - an Approved notice for the purpose of


(危險告示) identifying areas where unauthorised
approach is dangerous.

DANGER LIVE NOTICE - a notice in Approved form attached to


(帶電危險告示) indicate Live Apparatus adjacent to the
point of work, calling attention to the
Danger of approach to, or interference
with, such Apparatus.

DANGER TEST IN - a notice in Approved form placed at the


PROGRESS NOTICE boundary of the test area warning against
(試驗在進行中危險告示) unauthorised approach.

DEAD - at or about zero voltage and


(不帶電) disconnected from any Live system.

DECOMMISSIONING - a document issued to notify all parties


NOTICE concerned that from a particular date
(停止使用告示) and time the Apparatus or Mechanical
Plant is disconnected from all sources
of supply and is no longer subject to
the provisions of the Company's Safety
Rules.

EXTRA LOW VOLTAGE - normally not exceeding 50V a.c./120V


(特低壓) d.c., whether between conductors or to
earth.

HIGH VOLTAGE - exceeding 1000V a.c./1500V d.c. between


(高壓) conductors or 600V a.c./900V d.c. to
earth.

IMMEDIATE - shall mean under the supervision of a


SUPERVISION Competent Person at the point of work.
(現場監督)

IONISING RADIATION - any radiation capable of producing


(電離輻射) ionisation, commonly those associated
with alpha particles, beta particles,
protons, neutrons, X-rays and gamma
rays, all of which may be used for the
purpose of non-destructive testing.

ISOLATED - to separate and secure Apparatus or


(隔離) Mechanical Plant from any mechanical,
electrical or residual energy source.

LIMITATION-OF-ACCESS - a document issued by an Authorised


(工作範圍證) Person to a Competent Person defining
the limits and nature of work which may
be carried out in the vicinity of
Mechanical Plant. A Limitation-of-
Access shall be issued when other safety
documents are not applicable, and verbal
instructions are not considered
sufficient.

LIVE - electrically charged.


(帶電)

LOCK-OUT BOX - a device for the secure retention of all


(安全匙箱) keys used to lock means of isolation,
earthing or other safety devices
necessary for the issue of Safety
Documents. It may consist of a box or
other device fitted with a number of
locks of a type operated by non-
interchangeable keys. In addition, one
lock of each Lock-out Box shall be of a
type that can be operated only by a key
in the possession of an Authorised
Person.

LOST SAFETY DOCUMENT - a declaration issued by the General


DECLARATION Manager responsible for the operation
(遺失安全文件聲明) and maintenance of the Apparatus or
Mechanical Plant concerned to notify all
involved parties that a Safety Document
has been lost and is to be regarded as
cancelled.
LOW VOLTAGE - normally exceeding 50V a.c./120V d.c.
(低壓) between conductors, or to earth, but not
exceeding 1000V a.c./1500V d.c. between
conductors or 600V a.c. /900V d.c. to
earth.
MECHANICAL PERMIT- - a form of declaration signed and issued
TO-WORK by an Authorised Person to a Competent
(機械工作許可證) Person in charge of work to be carried
out on Mechanical Plant. The
declaration shall make known to such
person exactly what plant is Isolated
from all energy sources and on which
work may be carried out.
MECHANICAL PLANT - includes all prime movers, mechanical
(機械設備) parts of generators, pumps, pressure
vessels and associated pipework, cranes,
combustion chambers and passes of oil,
gas or coal fired boilers, storage tanks
and circulating water systems.
MINIMUM SAFETY - the clearance which must be maintained
CLEARANCE at all times between any person,
(最小的安全距離) including any object he may be carrying,
and exposed Live High Voltage conductors
to avoid Danger.
NATURAL GAS - a mixture of naturally occurring gaseous
(天然氣) hydrocarbons and non-hydrocarbons. The
main constituent is methane. A mixture
of Natural Gas of approximately 4% to
15% by volume in air is highly
flammable. Natural Gas is usually
lighter than air and will rise when
released into atmosphere. Natural Gas is
colourless, odourless and is not toxic
but it can cause suffocation due to lack
of oxygen.
NATURAL GAS - an area in which special precautions must
CONTROLLED AREA be taken as Natural Gas may be present.

NATURAL GAS - a certificate of declaration issued by an
INTERCONNECTION Authorised Person or a responsible
PIPELINE ISOLATION official of Guangdong Dapeng LNG Company
CERTIFICATE Limited (GDLNG). The certificate shall
(天然氣連接喉管隔離證書) state the isolation applied and other
precautions taken on the Natural Gas
interconnection pipeline. The issuing
party shall send a copy of the
certificate to the other party for their
preparation of Safety Documents on the
Natural Gas interconnection pipeline.

NATURAL GAS PIPELINE - shall mean parts of those physical


(天然氣喉管) facilities that are used for containment
of Natural Gas for the purpose of
conveying such gas to and from the
Natural Gas Plant.

NATURAL GAS PLANT - shall mean parts of those physical


(天然氣設備) facilities normally containing Natural
Gas for the purpose of pigging, metering,
heating, venting, regulating, treating or
other purposes including, but not limited
to pig receivers, metering units, gas
heaters, flare system, pressure control
system, regulator stations, valves,
instruments, equipment, protection system
and any other fixed attachments.

NATURAL GAS SAFETY -a document which describes the


MANUAL requirements and standards that have to
(天然氣安全手冊) be followed in relation to the Natural
Gas system.

SAFETY CODE OF - an Approved procedure detailing safe


PRACTICE working methods and precautions necessary
(安全操作守則) for a particular work requirement. A
Safety Code of Practice shall be issued
with the approval of the relevant
Divisional Head, and shall serve to
supplement the provisions of the Safety
Rules.

SAFETY DOCUMENT - a Mechanical Permit-to-Work, Limitation-


(安全文件) of-Access, Confined Space Permit-to-Work,
or Sanction-for-Work issued to a
recipient. In this copy of Safety Rules,
only a specimen of each of the bilingual
Safety Documents is included. English or
Chinese Safety Documents may also be used
provided their formats have been approved
by the Safety Rules Committee.

SAFETY DOCUMENT - a form attached to a Safety Document


ISOLATION listing the isolation points and actions
CONTINUATION SHEET taken to avoid Danger when the writing

Safety Document Isolation Continuation
Sheet which shall be signed by an
Authorised Person who prepares the Safety
Document and is part of the Safety
Document.

SAFETY KEY - a key supplied with a Safety Lock.

SAFETY LOCK - a lock supplied with a unique key, used


(安全鎖) to secure means of isolation, earthing,
or other safety devices necessary for the
issue of a Safety Document.

SAFETY RULES - a document issued to notify all parties


INITIATION concerned that from a particular date and
CERTIFICATE time, the Mechanical Plant must be
(安全規章生效證書) regarded as having been put into service
and subject to the provisions of the
Company's Safety Rules.

SANCTION-FOR-WORK - a form of declaration signed and issued


(核准工作證) by an Authorised Person to an Authorised
Person in charge of the work on
Mechanical Plant. The declaration shall
make known to such person exactly what
Mechanical Plant is to be worked upon and
the Safety Code of Practice in accordance
with which such work is to be conducted.

SCREEN - an Approved means of identifying or


(屏障) shielding the safe working area from a
source of Danger.

SENIOR CHEMIST - an Authorised Person who possesses a


(高級化學師) valid Certificate of Qualification to
carry out specific operations or work on
Mechanical Plant.

SPECIFIED GAS WORK - work in relation to the design,


(指明氣體工作) construction, maintenance, operation and
emergency handling of Natural Gas
Pipeline or Natural Gas Plant which
requires a specific level of competency
in accordance with the Natural Gas Safety
Manual.

SYSTEM OPERATING -a document which describes the


PROCEDURE responsibilities and duties of persons
(系統操作程序) who are involved in the Company’s High
Voltage system. It also contains
parameters and standards which have to be
referred to and procedures that must be
followed by those persons when carrying
out their duties.

WORKING PARTY - the persons working under the supervision


(工作小組) of a Competent Person or Authorised
Person.

5.
DANGEROUS OCCURRENCES AND REPORTABLE INCIDENTS

All dangerous occurrences and reportable incidents on the


Company's Mechanical Plant or premises shall be reported
immediately to the appropriate Control Engineer.

6.
ADMITTANCE TO OPERATIONAL AREAS

Admittance to operational areas shall be in accordance with local


standing instructions.

7.
FAILURE OF SUPPLY

A failure of supply, from whatever cause, to or from any part of


the High Voltage system shall be immediately reported to the
appropriate Control Engineer. During a failure of supply all
Apparatus shall be regarded as Live until Isolated.

8.
VARIATION OF RULES

In exceptional circumstances, these Safety Rules may be varied to


such extent as may be authorised in writing by the Managing
Director of the Company or person nominated by him in writing.

9.
TREATMENT FOR ELECTRIC SHOCK AND USE OF FIRST AID EQUIPMENT

All persons who may be concerned with the operation of, and work
on, the Company's system and Apparatus shall make themselves
conversant with the treatment for electric shock. Information
regarding treatment is given in Appendix 'C'.

The first aid equipment provided by the Company must not be used
except for the purposes intended. All persons should be familiar
with the location of the first aid equipment appropriate to their
place of working, particularly those in locations exposed to
accident from dangerous chemicals or by burning.

First aid boxes shall be properly maintained.

10.
GENERAL WARNINGS AND INSTRUCTIONS
10.1
Use of Safety Belts and Safety Harness

All persons whilst working on towers, high structures, platforms


or staging without suitable rails or fences, shall wear and make
proper use of Approved safety belts or safety harnesses. No
person shall work alone in any situation where safety belts or
safety harnesses are required to be worn.

10.2
Scaffolding and Staging

All scaffolding and staging shall be inspected by a competent


person as specified in Government Regulations before work is
allowed to commence. Subsequent inspections shall be carried out
regularly in accordance with the appropriate Safety Code of
Practice.

10.3
Ladders

All ladders shall be soundly constructed, and properly maintained


and regularly inspected. Before use, ladders shall be inspected
and measures taken to ensure security of the ladder by footing,
lashing or other means, in accordance with the appropriate Safety
Code of Practice.

10.4
Guarding of Walkways, Stairways and Platforms

Walkways, stairways and platforms when not otherwise guarded


shall have a handrail approximately one metre in height and a
midrail. Walkways and platforms in working areas shall not be
less than 400mm wide and shall be provided with toeboards of at
least 50mm in height and in line with the relevant ordinance.

10.5
Guarding of Holes

All openings in floors shall be securely fenced except when the


nature of the work in progress renders such fencing impracticable,
under these conditions, other suitable means of avoiding Danger
shall be provided.

10.6
Use of Safety Helmets

No person shall enter a designated hard hat area or construction


site unless he is wearing an Approved safety helmet.

10.7
Use of Ear Protectors

No person shall remain or work in designated high noise areas for


periods longer than those specified in Government Regulations
unless he is wearing Approved ear protectors for the noise level
involved.

10.8
Use of Eye Protectors

All persons engaged in work such as grinding, welding and


handling of dangerous chemicals, shall make proper use of
Approved goggles or other Approved means to protect the eyes.

10.9
Use of Breathing Apparatus

Breathing apparatus shall only be worn by suitably trained


personnel.

10.10
Guarding of Machinery

No person shall wilfully remove or interfere with any guard or


safety device provided for the purpose of securing safety.

10.11
Safety Barriers

Barriers (including plates on access ladders to operating


platforms) cutting off access to chambers containing dangerous
Mechanical Plant must always be kept locked, except when opened
for the purpose of giving admittance to Authorised Persons or
persons acting under their Immediate Supervision.

10.12
Lagging of Pipes

All accessible pipework the surface temperature of which is liable to exceed a temperature of
60oC shall be lagged or screened.

10.13
Compressed Gas Cylinders

Compressed gas cylinders shall be handled with care, kept as cool


as possible and free from oil or grease. They shall be used only
in a well ventilated position and stored in a legally prescribed
type of enclosure. Appropriate Government Ordinances with respect
to colour, identification and validity period shall be complied
with.

10.14
Ionising Radiations

(a) A site inspection shall be carried out by the Authorised


Person, Competent Person in charge of the work and the
qualified person from the non-destructive testing (NDT)
contractor to determine the extent of access and the
precautions necessary to permit the work to be conducted
safely.

(b) The qualified person from the NDT contractor shall, in the
presence of the Authorised Person and the Competent Person in
charge of the work, identify the proposed boundary of his
working area by the placement of Barriers and/or notices.
The Barriers/notices shall remain in position to prevent
unauthorised approach to the area throughout the period of
testing.

(c) The Authorised Person shall issue an appropriate Safety


Document to the Competent Person in charge of the work
clearly detailing the work requirement.

(d) All work shall be carried out in accordance with the


appropriate Safety Code of Practice.

10.15
Lost Safety Documents

In situations where a Safety Document has been deemed to be lost,


the circumstances of its loss will be properly investigated and
its cancellation approved in accordance with the appropriate
Safety Code of Practice.

10.16
Fire Protection Systems

10.16.1
Access into Areas Protected by Automatic Fire Protection Systems
of the Total Flooding Type (Water or Gas)

(a) When work is to be carried out on any Apparatus or equipment


protected by fixed fire-fighting systems, or where access
into any area protected by such systems is required, then if
the system is likely to present a Danger to those working, it
shall be Isolated or selected to manual control.

(b) Where it has been necessary to Isolate or select to manual


control an automatic fire protection system, it shall be
restored to fully automatic operation immediately after the
personnel engaged on the work or inspections have withdrawn
from the protected area or zone.

(c) Where fixed fire-fighting systems have been Isolated and


locked off either as part of the isolation applied to the
Apparatus or equipment to be worked upon, or where work is to
be carried out on the fire-fighting system itself, the keys
controlling the isolation shall be placed in a Lock-out Box,
Caution Notices fixed, and details recorded on the Safety
Document.

(d) Where fixed fire-fighting systems have been selected to


manual control to allow access into a protected area or zone
but remain available for service if required, this shall be
recorded on the Safety Document and "Caution Fire Protection
System On Manual Control For Access" notices shall be applied
to all points where the system has been selected to manual
control.

10.16.2
Use of Fire Hoses

Fire Services Department or other personnel using fire hoses in


the vicinity of exposed Live High Voltage conductors should be
warned of the Dangers. Under no circumstances must a solid jet be
directed at exposed Live High Voltage conductors, or be allowed
to approach to within 3 metres of them. Water jets should be
brought under control pointing away from the Live High Voltage
conductors.

10.16.3
Operation of Gas or Chemical Extinguishers

(a) Carbon Dioxide (CO2) Gas

Carbon dioxide is an asphyxiant. If such a system has


operated, or if portable CO2 extinguishers have been
discharged into any enclosed, poorly ventilated or low lying
area, no access into the area must be permitted until it has
been confirmed safe to do so.

(b) BCF, BTM and FM 200 Gas

All these gases are classified as mildly toxic. If such a


system has operated, or if portable extinguishers of these
gases have been discharged into any enclosed poorly
ventilated or low lying area, no access into the area must be
permitted until it has been thoroughly ventilated.

(c) Foam

Portable foam extinguishers must not be used on High, or Low


Voltage Apparatus unless such Apparatus has been made Dead.

(d) Dry Powder

Dry powder fire extinguishers are suitable for all fires


including Live electrical and liquid fires. In all cases the
Minimum Safety Clearance must be maintained.
10.17
Pressure Testing of Vessels, Receivers and Pipework

All such testing shall be conducted in accordance with the


appropriate Safety Code of Practice and shall wherever
practicable be carried out using hydraulic means.

10.18
Handling of Asbestos

Any asbestos process is subject to stringent Government


Regulations and must be conducted fully in accordance with them.
In order to adequately cover safety requirements for work of
differing nature and complexity, all such work will be conducted
in accordance with the appropriate Safety Code of Practice.

11.
LANGUAGE REQUIREMENT

The Company accepts both English and Chinese as the official


languages for the purposes of communication in connection with
all operations conducted in accordance with these Rules.

SECTION 2
SAFETY RULES FOR WORK ON MECHANICAL PLANT

M.1
PUTTING INTO SERVICE OF NEW MECHANICAL PLANT

M.1.1
Preparation for Service of New Mechanical Plant

A Safety Rules Initiation Certificate shall be issued on any item


of Mechanical Plant prior to it being put into service.

The Safety Rules Initiation Certificate shall be issued in


accordance with the following procedure:-

(a) All parties concerned shall acknowledge that the Mechanical


Plant is to be put into service from the time and date
indicated by signing all copies of the Safety Rules
Initiation Certificate. Each involved party shall retain his
copy for reference purposes and return all other copies to
the sender.

(b) A copy of the completed Safety Rules Initiation Certificate


bearing the names of all departments or contractors notified,
together with diagrams if necessary, shall be in the
possession of the appropriate Control Engineer before the
Mechanical Plant is put into service.

After the stated time and date of putting into service in the
Safety Rules Initiation Certificate, all further work shall be
carried out in accordance with these Rules.

M.1.2
Initial Operation of the Mechanical Plant

Before any Mechanical Plant is put into service, the following


provisions shall be met:-

(a) Rule M.1.1 must be complied with.

(b) All essential pre-commissioning check lists necessary to


ensure safety must have been completed.

M.2
GENERAL SAFETY PRECAUTIONS

M.2.1
Work on Mechanical Plant

(a) No person shall start work on any part of Mechanical Plant


until such parts have been Isolated and rendered free from
all sources of Danger, locked off and have Caution Notices
affixed to all points of isolation by a Competent Person. An
Authorised Person shall satisfy himself that these
requirements are fully complied with before he issues a
Mechanical Permit-to-Work.

(b) Where work is to be carried out on Mechanical Plant that is


electrically driven or operated, the switch or isolators
controlling the supply to the electrical drive shall be
locked open. Where the point of isolation to the electrical
drive is provided by the withdrawal of fuses, empty fuse
receptacles shall have Caution Notices applied suitably
inscribed with the Safety Document number.

(c) Lock-out Boxes shall be provided for the retention of Safety


Keys or removed fuse carriers used to secure the isolation
of the Mechanical Plant.

(d) Providing the work on Mechanical Plant is such that it may


be carried out safely with Mechanical Plant in service, such
as cleaning, adjustment and painting, the Authorised Person
may at his discretion issue a Limitation-of-Access, and the
provisions of Rules M.2.1 (a), (b) and (c) shall not apply.

M.2.2
Fixing of Notices, Barriers and Screens

An Authorised Person shall satisfy himself that Caution Notices


are properly placed. Danger Notices, Barriers or Screens shall be
fixed or moved only under the Immediate Supervision of the
Competent Person in charge of the work.

M.2.3
Work Requiring the Operation of the Prime Mover

Where work on Mechanical Plant requires the operation of the


prime mover, this work shall be conducted strictly in accordance
with the appropriate Safety Code of Practice. A Sanction-for-Work
quoting the appropriate Safety Code of Practice number shall be
issued for the work.

The Sanction-for-Work together with the Safety Key(s) used to


isolate the prime mover, shall be issued to an Authorised Person
specifically trained in such work and holding a valid Certificate
of Qualification to this effect.

The work requiring the operation of the prime mover shall be


carried out under the Immediate Supervision of the Authorised
Person receiving the Sanction-for-Work, who shall be responsible
for ensuring that safe working procedures are adopted throughout.
On completion of the work, the prime mover will be re-isolated
and the Sanction-for-Work and Safety Key(s) returned for
cancellation.

M.2.4
Mechanical Permit-to-Work

No work other than the following exceptions may be carried out on


Mechanical Plant unless a Mechanical Permit-to-Work has been
issued by an Authorised Person to the Competent Person
supervising such work.

Exceptions

(a) Work specified in Rules M.2.1(d) and M.2.3.

(b) Work carried out using the "Furmanite" process.

(c) Floating and adjusting of safety valves.

(d) Tightening of joints or tightening of glands on


valves or pumps.

The Competent Person receiving the Mechanical Permit-to-Work


shall be deemed to be the person in charge of the work, he shall
remain on site whilst work is in progress and shall be
responsible for ensuring that all members of the Working Party
comply with the requirements of the Safety Rules.

A duplicate of the Mechanical Permit-to-Work shall be kept in the


office of the generation Control Engineer or other appointed
place and the safety precautions shall not be withdrawn until the
Mechanical Permit-to-Work has been cancelled.

M.3
PRESSURE RECEIVERS AND ASSOCIATED PIPEWORK

The Mechanical Plant shall not be released for work by the issue
of a Mechanical Permit-to-Work until it has been:-

(a) Isolated from all sources of supply including common


blowdown;

(b) brought to atmospheric pressure and if necessary drained;

(c) wherever possible have drains and vents locked open to


atmosphere.

A non-return valve shall not be considered as a shut off valve


for the purpose of isolation unless it is capable of being locked
shut.

Where work is to be carried out using the "Furmanite" process,


approval for the work shall be sought from the appropriate
Divisional Head or his deputy. A Limitation-of-Access shall be
issued to the Competent Person in charge before work commences.

M.4
PULVERIZED FUEL, GAS & OIL FIRED BOILERS & ANCILLARY EQUIPMENT

M.4.1
General Provisions

Operation of the boiler and its associated firing systems shall


be in strict compliance with current editions of operating
instructions and the pulverized fuel Safety Code of Practice.

M.4.2
Work in Combustion Chambers, Gas Passes and Dead Spaces

Before a Confined Space Permit-to-Work is issued to allow access


into a boiler combustion chamber, gas pass or dead space the
following precautions should be taken as appropriate:-

(a) All means of firing the boiler should be Isolated and locked.

(b) All supplies which enable the movement of oil burners, coal
burners or windboxes should be Isolated and locked.

(c) All sources of draught to and from the boiler should be


Isolated and locked with the exception that, at the
discretion of the Authorised Person issuing the Confined
Space Permit-to-Work, such fans as are necessary to assist
cooling or ventilation may be kept in service.

(d) All dampers, retractable temperature probes, sootblowers or


air-heaters in the working zone should be Isolated and
locked.

(e) Chemical injection systems for flue gas treatment should be


Isolated and locked.

(f) Ash and dust hoppers should be emptied or means taken to


prevent access to, the hoppers.

(g) The provisions of Rule M.13 will apply.

Where preparatory work such as removal of slag or dust build up


is required, this should be completed and the associated Confined
Space Permit-to-Work cleared and cancelled before other work is
allowed to proceed.

A Confined Space Permit-to-Work for access into a dead space


should draw attention to the requirement to check for dust banks.
Such checking is the responsibility of the Competent Person to
whom the Confined Space Permit-to-Work is issued and where
necessary, he should arrange for the removal of such dust.

Where work within the boiler is extensive and involves several


Working Parties, the work should be pre-planned and arranged with
an Authorised Person to ensure that the work of any Working Party
does not create a Danger to any of the other Working Parties.
Changes to such plans must be fully discussed with the Authorised
Person and agreed upon before they are implemented.

M.4.3
Operating Adjustments of Dampers, Retractable Temperature Probes
and Sootblowers

Where stroke checking or adjustments are to be carried out on


dampers, sootblowers, retractable temperature probes or any other
item whose movement could cause Danger, this will only be carried
out when no other work is taking place in the vicinity. Rule
M.2.3 will apply in addition to Rule M.13 and the Authorised
Person in charge of the work will be responsible for ensuring
that all unauthorised approach to such equipment is prevented.

M.4.4
Precautions on Completion of Work

Upon completion of work, the Confined Space Permit-to-Work holder


will ensure that all persons, equipment and tools under his
control are removed.

The final closing of all access doors will be carried out under
the Immediate Supervision of a Competent Person who will carry
out a final check that no person, equipment or tools are left
inside the boiler. He shall be accompanied by the Authorised
Person who is to cancel the final Confined Space Permit-to-Work.

M.4.5
Pulverized Fuel Mills & Pipework

M.4.5.1
Conditions For Access Into Coal Mills

(a) Where practicable all coal shall be run off, the mill purged
and cooled.

(b) All electrical supplies to the mill group shall be Isolated.

(c) The plant shall be Isolated from fuel and air supplies and
from the furnace by efficient dampers or blanks.

(d) The provisions of Rule M.13 will apply.

(e) If a mill containing coal or pulverized fuel has been


Isolated, a Confined Space Permit-to-Work shall be issued
for the additional work of removing the residual coal from
the mill system before entry for other work is permitted.
The coal and pulverized fuel mixture shall be removed with
the minimum disturbance to the pulverized fuel.

(f) Any suggestion of fire inside a mill must be handled in


accordance with the current edition of the pulverized fuel
Safety Code of Practice.

M.4.5.2
Work on Pulverized Fuel Pipework

For work on pulverized fuel pipework, the associated mill group


shall be Isolated and the pipework shall be Isolated from the
furnace by efficient dampers or blanks.

If a blank has to be fitted with the boiler on load, this shall


be carried out in accordance with the appropriate Safety Code of
Practice.

M.5
ACCESS TO BUNKERS AND SIMILAR SITUATIONS

Entry to any bunker, silo, pit or hopper for the purpose of


manual trimming is prohibited.

Entry for the purpose of final clearing out, maintenance or


inspection shall only be permitted under the terms of a Confined
Space Permit-to-Work and the following conditions shall then
apply:-

(a) Every effort shall be made to make safe the area by


dislodging any build up of coal, ash or dust that may
present a Danger of collapse. Where this is not possible,
precautions shall be taken to prevent approach to such areas.

(b) Unless provided with a suitable working platform, the member


of the Working Party entering the area shall wear an
Approved safety harness. The lines attached to the safety
harness shall be of Approved type and properly secured and
kept taut whilst he is inside by another member of the
Working Party stationed outside.

(c) All members of the Working Party shall be fully conversant


with the work requirements and the manner in which safety
line and safety harness are to be used to secure safety.

(d) The provisions of Rule M.13 complied with.

M.6
MAINTENANCE OF BELT CONVEYORS

M.6.1
General Requirements

Conveyors must not be operated until:-

(a) all guards are in place;


(b) all safety trip features are operable; and
(c) an audible warning has been sounded throughout the conveyor
before it is started.

Riding on conveyor belts is prohibited. Unless a conveyor belt is


Isolated under the terms of a Mechanical Permit-to-Work, no person
is permitted to step onto a conveyor table or cross a conveyor
except by walkways or other means provided for such purpose.

M.6.2
Maintenance of Belt Conveyors

With the exception of tracking adjustments or belt renewal, which


may be carried out under Rule M.2.3, no maintenance will be carried
out on belt conveyors unless a Mechanical Permit-to-Work has been
issued.

The Competent Person in charge of the work must ensure that Danger
cannot arise due to the release of stored energy. Clamping or
similar action may be necessary to prevent movement of the belt and
special arrangements may be needed to secure gravity take-up
pulleys and weights.

M.6.3
Clearing Spillage and Build Up

Clearing of spillage and the removal of build up must not be


carried out when the belt is in motion. The Competent Person in
charge of the work must ensure that adequate precautions are taken
to avoid Danger before allowing such work to commence.

If it is necessary to remove guards to carry out cleaning, both the


removal of the guards and the cleaning shall be done under the
terms of a Mechanical Permit-to-Work.

M.7
HYDROGEN COOLED GENERATORS
Flames and use of ignition sources are prohibited at any time
within, or near, the hydrogen control cabinet or cubicle unless
risk assessment has been conducted, the hydrogen system has been
purged and the local area proved free of flammable gas. Where it is
necessary to use equipment which could be an ignition source near a
hydrogen system, its use shall be confined to areas which have been
proved safe by an appropriate test procedure.

Smoking is prohibited within, or near, the hydrogen control cabinet


or cubicle.

Before any work is commenced which involves the opening up of any


part of a casing which normally contains hydrogen, the hydrogen
supply shall be disconnected from the hydrogen system and the
casing shall be cleared of hydrogen and completely scavenged by CO2
or other inert gas which shall in turn be replaced by air. A
relevant Safety Document, which shall certify that the casing is
free from all hydrogen and scavenging gas, shall then be issued. An
Approved detector shall be used as the basis for certification.

When work external to a hydrogen cooled machine, such as the


external maintenance of hydrogen seals, has to be carried out, the
casing shall be cleared of hydrogen and replaced by CO2 or other
inert gas. It is not necessary in these circumstances to replace
the CO2 or other inert gas by air, provided that the following
conditions are observed:-

(a) no part of the casing is dismantled; and

(b) the pressure in the system is maintained above atmospheric


pressure at all times.

Before work is commenced on any other part of the plant which


contains hydrogen, that part shall be Isolated from all sources
of hydrogen and the valves locked. Vessels and associated
fittings containing hydrogen shall, wherever practicable, be
opened to atmosphere and thoroughly ventilated. A relevant Safety
Document, which shall state the precautions taken to clear that
part of the plant of hydrogen shall then be issued.

M.8
CHEMICAL SAFETY

M.8.1
General Requirements

The following precautions shall be taken when handling any


chemical substance:-

(a) All chemicals shall be considered as being a potential


hazard and constituting a Danger. Before and during their
use all appropriate Safety Codes of Practice shall be
observed.

(b) All chemicals when kept in quantities greater than exempted


in the "Dangerous Goods Ordinance" shall be stored in an
appropriate "Dangerous Goods Store" as required by the
Ordinance. No person except an Authorised Person or Senior
Chemist or persons nominated by an Authorised Person or
Senior Chemist shall have access to any such store.

(c) All chemicals must be kept in containers of adequate


strength and appropriate material. Badly damaged containers
shall not be accepted on delivery and if damaged in use,
shall be emptied prior to disposal.

(d) All chemical containers shall be clearly identified.

(e) Areas where chemicals are being used shall be cordoned off
and warning notices posted in English and Chinese, unless
considered unnecessary by a Senior Chemist.

(f) Adequate protective clothing shall be worn when deemed


necessary and any procedures regarding the use of protective
clothing contained in the appropriate Safety Codes of
Practice shall be observed.

M.8.2
Handling Corrosive Acids and Strong Alkalies

In addition to any procedures required in the appropriate Safety


Codes of Practice, the following precautions shall be taken when
handling corrosive acids and strong alkalies:-

(a) Full protective clothing shall be worn.

(b) When using corrosive acids, an ample supply of water and


sodium bicarbonate, or other antidote, shall be on hand in
case of accident.

(c) When using strong alkalies, an ample supply of water and


acetic acid, or other antidote, shall be on hand in case of
accident.

(d) The work shall be done under the supervision of a Senior


Chemist or person nominated by a Senior Chemist.

M.8.3
Handling Cryogenic Fluids
Risk assessment shall be conducted before handling Cryogenic
Fluids. In addition to any procedures detailed in the appropriate
Safety Code of Practice, the following precautions shall be taken
when handling Cryogenic Fluids, e.g. liquid nitrogen:-
(a) The area where Cryogenic Fluids are in use shall be cordoned
off. Smoking is prohibited within the cordoned area and "No
Smoking" signs shall be displayed.
(b) Flames and use of ignition sources shall be prohibited
within the cordoned area.
(c) Dry leather gauntlets which must be loose fitting, and an
Approved face visor shall be worn during the operation.
(d) Adequate ventilation of the working area shall be maintained
at all times.
(e) No part of the body shall be allowed to touch any
uninsulated pipes or vessels containing Cryogenic Fluids.
(f) Cryogenic Fluids shall only be put into vessels which are
specifically designed for their use. The use of any other
vessel is prohibited.

M.9
PRIME MOVERS

M.9.1
General Precautions

Before work is carried out on any type of prime mover, the source
of energy and Mechanical Plant or generator to which it is
coupled shall be suitably Isolated and locked off to prevent the
system from rotating. Exceptions to this general requirement are
as follows:-

(a) If the nature of the work is such that operation of the


prime mover is necessary, this work shall be conducted in
accordance with Rule M.2.3.

(b) If the prime mover is equipped with barring gear, then work
with the system barring shall be at the discretion of the
Authorised Person issuing the Safety Document.

M.9.2
Internal Combustion Engines

In addition to the requirements of Rule M.9.1, where the prime


mover is an internal combustion engine the following conditions
shall apply:-

(a) The fuel supply shall be Isolated and locked off.

(b) The starting device shall be Isolated and locked off.

(c) Ignition systems shall be Isolated and locked off and in the
case of high energy ignition systems, discharged where
necessary to prevent Danger.

(d) Precautions shall be taken to prevent Danger associated with


common exhaust systems.

(e) Engine cleaning systems shall be Isolated and locked off.

M.10
WORK ON MECHANICAL PLANT CONTAINING OR OPERATED BY COMPRESSED AIR
OR GAS

M.10.1
Plant Containing or Operated by Compressed Air

The basic principles of locking shut compressed air supply valves,


reducing the pressure in the plant to atmospheric, and locking
open vent and drain valves where possible, will apply before a
Mechanical Permit-to-Work involving any of the following is
issued:-

(a) Work involving the opening up of any plant containing


compressed air.

(b) Work on plant operated by compressed air where Danger could


arise during the course of work because of stored energy in
the operating mechanism.

(c) Work where a point of isolation is operated by compressed


air.

M.10.2
Plant Containing or Operated by Compressed Gas

Before work is carried out which involves opening up any


Mechanical Plant containing or operated by compressed gas, the
gas supply valves shall be locked shut. The gas shall then be
removed and replaced by air at atmospheric pressure.

Where direct exchange of the gas with air could involve passing
through explosive regions, the gas shall first be purged using an
inert gas which will then be replaced by air.

Adequacy of purging shall be confirmed at each gas exchange using


an Approved detector.

M.10.3
Operating Adjustments

Operating adjustments which require the normal compressed air or


gas supply shall only be carried out under the supervision of an
Authorised Person and the provisions of Rules M.10.1 & M.10.2
will not apply.

M.11
POWER DRIVEN LIFTING MACHINERY, CRANES AND LIFTING TACKLE

(a) All cranes and lifting tackle shall be adequately maintained


and regularly inspected and a record of such inspections
kept.

(b) No person shall climb on to, crane, or lifting machine, or


crane rail without the knowledge of the driver operating the
crane or lifting machine.

(c) No person shall work on or near the wheel track of an


overhead travelling crane in any place where he would be
liable to be struck by the crane unless effective measures
have been taken, by warning the driver of the crane and
applying rail clamps, to ensure that the crane does not
approach within 6m.(20 ft) of that place and a Mechanical
Permit-to-Work has been issued before work commences.
Similar precautions shall be taken in connection with work
on or near the wheel track of every hoist or other lifting
machine.

(d) None of the moving parts of a crane, hoist or other lifting


machine shall be dismantled unless all parts of the machine
are so secured or placed as to remain stable. All rotating
machinery, driving gears, chains or belts shall be suitably
guarded or fenced off to prevent Danger.

M.11.1
Work on Coal Ship Unloaders

Wire rope changes are a routine maintenance function. Work must


be carried out in accordance with the appropriate Safety Code of
Practice covering ship unloader wire rope change and inspection
of ropes.

The Authorised Person in charge of the work must ensure all work
above ground level is carried out safely, taking into account the
effects of adverse weather conditions.

M.12
CIRCULATING WATER SYSTEMS

M.12.1
General Requirements

No person shall enter nor work be carried out in any culvert,


pipeline or associated pump until:-

(a) The culvert, pipeline or associated pump is Isolated from


all sources of water, drained and vented to atmosphere and
supplies of compressed air and chemical injection are, where
necessary, Isolated;

(b) The supply to motors operating all valves, intake gates or


penstocks are Isolated and locked off, all hand operated
mechanisms locked and the power supply to hydraulically
operated valves is shut off and locked;

(c) The provisions of Rule M.13 complied with.

M.12.2
Work on Circulating Water Systems

During dewatering, provision shall be made to prevent


unauthorised entry at each point where the system has been opened
up.

When the provisions of Rule M.12.1 have been complied with entry
will be restricted to working parties consisting of not less than
two persons. Precautions shall be taken to prevent or warn
against unauthorised entry.

Upon completion of work a final inspection will be carried out to


ensure that all personnel, tools and materials have been removed.
During this inspection persons will be stationed at each point of
access to prevent unauthorised entry and they shall remain at
station until the access is sealed.

M.13
CONFINED SPACES

M.13.1
Dangers Associated With Confined Spaces

The main Dangers to personnel working in a Confined Space are:-

(a) Asphyxiation.

(b) Poisoning due to toxic atmosphere such as carbon monoxide


which affects the human system so quickly that escape is
rarely possible.

(c) Burns from fire or explosion caused by the presence of


flammable gas or excess oxygen.

(d) The effect of chemical deposits on the human system, e.g.


due to bodily contact.

(e) The effects of abnormal temperatures.

(f) The effect of excessive noise.

(g) Falling.

Danger due to (a), (b) and (c) will be controlled if a sufficient


forced ventilation is provided such that:-

i. Exposure to toxic substances does not exceed the threshold


limit value and the concentration of flammable gases in the
Confined Space atmosphere is not above the lower flammable
limit,

ii. There cannot be a significant decrease or increase in the


amount of oxygen in the atmosphere.

At normal temperature and pressure the percentage of oxygen in


air by volume is approximately 21%. A lowering of this value may
result in the loss of consciousness by persons in such oxygen
deficient atmospheres. Under no circumstances should work be
allowed to proceed if the oxygen concentration falls below 18%.

M.13.2
General Provisions

The plant shall be Isolated from all sources of Danger and


Caution Notices applied at all points of isolation. Due
consideration shall be given to the work nature and process,
which could result in an initially safe working environment
becoming unsafe.

Where substances liable to give off gases, vapours or fumes (e.g.


some paints or resinous compounds) are to be taken into the space,
advice shall be sought from a Senior Chemist in order that
suitable precautions can be taken to avoid Danger.

Oxygen enriched atmospheres can cause normally quiescent


materials to become highly combustible. Care must therefore be
taken when using gas burning or welding equipment inside a
Confined Space to prevent the accidental escape of oxygen from
the equipment. If any gas is to be used inside a Confined Space
and its use is discontinued for more than one hour the gas supply
should be shut off at source.

Wherever practicable internal combustion engines should not be


used inside a Confined Space unless adequate precautions are
taken to remove the exhaust fumes. When used outside the space,
precautions should be made to prevent the exhaust fumes from
being drawn into the space.

M.13.3
Testing of Confined Spaces

Wherever possible the testing of a Confined Space should be


carried out from a position external to the space.

Where entry is necessary for testing this will be done under a


Confined Space Permit-to-Work and the following will apply:-

(a) Breathing apparatus shall be worn for entry into the space.

(b) Wherever practicable the person entering the space should


wear an Approved safety harness to which an Approved safety
line is securely attached and which is controlled by
personnel who will be stationed outside the space. Such
personnel shall monitor the progress of the person inside
the Confined Space and must be capable of pulling him out if
required. In addition a further person shall be stationed
outside the space with breathing apparatus ready for use.

(c) Sources or materials which could inadvertently cause


ignition must not be introduced into the Confined Space or
the vicinity of the opening of a Confined Space which has
previously contained a flammable substance. Particular
attention must be paid to the suitability of lighting
arrangements.

(d) Any sludge or deposits which might be disturbed by persons


when working must be stirred before samples are taken.

On completion of testing, a Confined Space Permit-to-Work will be


issued by the Authorised Person stating what testing has been
carried out and what special precautions are necessary for work
inside the space to be conducted safely.

M.13.4
Confined Spaces

No person shall be permitted to enter a Confined Space for the


purpose of work or inspection until a risk assessment report and
a Confined Space Permit-to-Work have been issued by an Authorised
Person to a Competent Person in charge of the Working Party. All

such work shall be carried out in accordance with the appropriate


Safety Code of Practice. The Working Party shall consist of not
less than two persons.
In the event of failure of any forced ventilation provided, it is
the responsibility of the Competent Person in charge of the work
to evacuate the persons working in the Confined Space as quickly
as possible.
Wherever practicable an Approved device for continuous monitoring
of the Confined Space will be used. It is the responsibility of
the Competent Person in charge of the work to ensure that
batteries for this device are replenished during the course of
work.
If the work is to be carried out using breathing apparatus this
will be used as laid down in Rule M.13.3 (b).
Where work in a Confined Space is permitted without breathing
apparatus and when a continuous monitor is not to be used
throughout the work, the interval that gas testing is required
should be stated on the Confined Space Permit-to-Work. If a
continuous monitor is in use, the Confined Space Permit-to-Work
shall be endorsed to the effect that should the alarm operate,
the Confined Space will be vacated until retested and a new
Confined Space Permit-to-Work issued.
M.14
VESSELS WHICH HAVE CONTAINED OIL, GAS OR OTHER FLAMMABLE
SUBSTANCES
Flames and use of ignition sources are prohibited in the vicinity
of open vessels containing, or which have contained, oil, gas or
any flammable substance unless risk assessment and adequate
precautions have been taken.
Work on such vessels involving the application of heat is
prohibited until an Authorised Person has taken all practical
steps to prevent fire or explosion either by removal of the
flammable substance and any fumes, or by rendering them non-
explosive and non-flammable.
Before any person is allowed to enter or commence work in a
vessel which has contained oil, gas or any other flammable
substance, the provisions of Rule M.13 must be complied with.
Smoking is prohibited in the vicinity of the open vessels
containing or which have contained oil, gas or any flammable
substance.
M.15
NATURAL GAS CONTROLLED AREAS, PIPELINES AND PLANTS
M.15.1
General Requirements

The following precautions shall be taken in Natural Gas


Controlled Areas and for work on Natural Gas Pipelines and
Natural Gas Plants:

M.15.1.1
Level of Competency for Specified Gas Work

Persons performing Specified Gas Work shall possess the
appropriate level of competency according to the Natural Gas
Safety Manual.

M.15.1.2
Access Control

Access to a Natural Gas Controlled Area shall be strictly


controlled according to the appropriate Safety Code of Practice.

Access to the flare-affected zone during flare operation is


prohibited.

M.15.1.3
Work Control

All work in a Natural Gas Controlled Area shall be carried out in


accordance with the appropriate Safety Code of Practice.

M.15.1.4
Earthing Connections on Natural Gas Pipelines and Natural Gas
Plants

Earthing connections on Natural Gas Pipelines or Natural Gas


Plants for the purpose of lightning protection or discharging
static electricity shall not be interfered with, unless special
provisions are provided to ensure the continuity of earthing.

M.15.2
Natural Gas Controlled Areas

Within a Natural Gas Controlled Area, different hazardous area


zones have been classified. The following classification is
according to the adopted international standards and in order of
decreasing probability of explosive gas/air mixtures being present.

(a) Zone 0 - place in which an explosive atmosphere consisting


of a mixture with air of flammable substances in
the form of gas, vapour or mist is present
continuously or for long periods or frequently.

(b) Zone 1 - place in which an explosive atmosphere consisting


of a mixture with air of flammable substances in
the form of gas, vapour or mist is likely to occur
in normal operation occasionally.

(c) Zone 2 - place in which an explosive atmosphere consisting


of a mixture with air of flammable substances in
the form of gas, vapour or mist is not likely to
occur in normal operation but, if it does occur,
will persist for a short period only.

The zoning shall be indicated with drawings and notices displayed


at prominent locations leading to the Natural Gas Controlled Area.
Where practicable, Zones shall be demarcated on-site.

Within the Natural Gas Controlled Area, design, construction,


operation and maintenance of equipment shall follow the
requirements stated in the Natural Gas Safety Manual.

Atmospheric testing shall be carried out regularly when there is


activity within the Natural Gas Controlled Area to ensure that
Natural Gas does not accumulate to a dangerous level in the
vicinity.

M.15.3
Ignition Sources

Before any person enters Zone 0 or Zone 1 area, the person must
discharge the static electricity, if any, on the person’s body by
physical contact to an earthed metal provided for the purpose.

Smoking is prohibited in a Natural Gas Controlled Area.

Only non-spark hand tools and only intrinsically safe or


flameproof equipment shall be used within Zone 0 or Zone 1 area
unless the plant which may contain Natural Gas is Isolated and
has been purged with nitrogen or other inert gas.

The use of communication, electrical or electronic equipment such


as electrical tools, torches, cellular phones, walkie-talkies,
pagers, personal digital assistants, cameras, computers,
instruments, etc. is not allowed in a Natural Gas Controlled Area
unless the equipment is flameproof or intrinsically safe. Where
it is necessary to use an item which could be an ignition source,
its use shall be confined to Zones which have been proved safe by
the appropriate test procedure.

Always use appropriate Approved portable test equipment to


monitor the atmosphere within a Natural Gas Controlled Area.

M.15.4
Welding or Flames

The use of any flames or welding equipment within any Natural Gas
Controlled Area is not permitted unless the safety precautions
stated in the appropriate Safety Code of Practice have been
complied with.

The use of a Natural Gas Pipeline as earthing connection for


electrical welding is prohibited.

M.15.5
Work On Natural Gas Pipelines

M.15.5.1
Open Up A Pipeline

Before opening up a Natural Gas Pipeline, the following


procedures shall be followed:

(a) The pipeline shall be Isolated. ‘Double block and bleed’


principle shall be applied for isolation from sources of
Natural Gas as far as is reasonably practicable. All
relevant isolation valves shall be locked shut with Caution
Notices attached.

(b) The pipeline shall be purged with nitrogen, other inert gas
or water and vented at a remote point to displace the
contained Natural Gas, in accordance with the appropriate
Safety Code of Practice.

(c) If the work is to be carried out in a Confined Space or an


unventilated space where an inert gas may accumulate, the
space must be purged in accordance with the appropriate
Safety Code of Practice.

(d) If the work is on a Natural Gas Pipeline having points of


supply controlled by a relevant gas control authority, a
Natural Gas Interconnection Pipeline Isolation Certificate
shall be issued in accordance with the appropriate Safety
Code of Practice.

(e) A relevant Safety Document, stating the precautions taken and


specifying the limits and boundary of the work, shall then be
issued.

Where the pipeline is housed indoors, whether doors and


ventilators should be opened or switched on during work shall be
stated in the Safety Document. Use of fans or blowers to assist
ventilation at work shall conform to the requirements stated in
the appropriate Safety Code of Practice. Approved gas monitor(s)
shall be used by the Working Party at the vicinity of work.

Before breaking a Natural Gas Pipeline installed above sea level,


a portable earthing lead having a cross section of not less than
6 square mm copper equivalent must be applied at both ends of the
proposed break to maintain the continuity of earthing. The
earthing lead shall remain firmly applied to metallic parts that
are free from paint or other insulated coating until the
component or pipeline is reinstated in accordance with the
appropriate Safety Code of Practice.

For a Natural Gas Pipeline installed below sea level, all work
shall be carried out in accordance with the appropriate Safety
Code of Practice.

M.15.5.2
Connections and Attachments

When welding, drilling or bolting on a Natural Gas Pipeline is


necessary, the appropriate Safety Code of Practice must be
followed.

M.15.5.3
Work Not Involving Opening Up of A Pipeline

Providing the work on a Natural Gas Pipeline can be carried out


safely with the Natural Gas Pipeline in service, such as cleaning,
adjustment and painting, without any surface preparation that
would generate sparks, the Authorised Person may at his
discretion issue a Limitation-of-Access for such work, and the
provisions of Rules M 15.5.1 (a), (b) and (d) shall not apply.

M.15.5.4
Work Completion

Before the re-admission of Natural Gas, the pipeline must first


be checked for integrity, including the earthing connections,
interlocks and protections; purged with nitrogen or other inert
gas, or pigging, to displace contained air; and checked to be
free of leakage in accordance with the appropriate Safety Code of
Practice.

All pipeline must be pressurized slowly and checked for gas


leakage.

M.15.6
Work on Natural Gas Plants

M.15.6.1
Open Up A Natural Gas Plant

Before opening up a Natural Gas Plant, the following procedures

M.15.6.3

generate sparks, the Authorised Person may at his discretion
issue a Limitation-of-Access, the provisions of Rules M 15.6.1 (a)
and (b) shall not apply.

M.15.6.4
Work Completion

Before the re-admission of Natural Gas, the plant must first be


checked for integrity, including the earthing connections,
interlocks and protections; purged with nitrogen or other inert
gas to displace contained air; and checked to be free of leakage
in accordance with the appropriate Safety Code of Practice.

The plant must be pressurized slowly and checked for gas leakage.

M.15.7
Natural Gas Venting

Checks and arrangements shall be made to ensure the process of


gas venting does not make other areas fall into a more hazardous
zone. Where it is not practicable to do so, special safety
arrangements shall be made.

Temporary vent pipe must be fitted to exhaust gas vertically and


at a height of at least 3 metres above the working level or the
ground level, whichever is higher. A flame arrestor shall be
provided at the vent outlet.

An Authorised Person shall conduct a risk assessment in


accordance with the appropriate Safety Code of Practice before
any manual venting of Natural Gas. The vent location must be free
from any activity that could provide a source of ignition. Where
necessary, any operating plant that may provide a source of
ignition within the Zone surrounding the vent shall be shut down
prior to the venting.

In the risk assessment, the prevailing wind direction must be


considered. No manual venting shall be allowed when lightning
warning is in force.

M.15.8
Vehicles

Vehicles shall not enter Zone 0 or Zone 1 area unless the plant
has been Isolated and purged with nitrogen or other inert gas.

Vehicles may be allowed to enter Zone 2 area with the approval of


an Authorised Person after conducting a risk assessment in
accordance with the appropriate Safety Code of Practice. Flame
arrestors shall be fitted to the exhausts of the vehicles before
the access is granted.

M.15.9
Excavations

The conditions set out in the relevant Safety Code of Practice,


government regulations and codes of practice must be strictly
followed for any excavation work in the vicinity of or over a
buried Natural Gas Pipeline. Where necessary, a Limitation-of-
Access shall be issued for the work. The Natural Gas Pipeline
must be checked and proved to be free of leakage before back-
filling.

M.15.10
Interlocks

Interlocks are provided for the specific purpose of protecting


the Natural Gas Plant. Only under exceptional circumstances
should steps be taken to defeat the function of an interlock.
Where it is required to defeat interlocks, this may only be
sanctioned by the person appointed by the Company to do so.

Only an Authorised Person will receive the sanction to defeat


Natural Gas Plant interlocks. When carrying out the operation,
the Authorised Person shall be accompanied by a Competent Person
who is familiar with the intended operation.

M.16
DECOMMISSIONING OF MECHANICAL PLANT

All work necessary to disconnect Mechanical Plant and render free


from all sources of supply will be conducted under the terms of
an appropriate Safety Document. On completion of such work, a
Decommissioning Notice shall be issued after which these Rules
shall not apply.

automatic fire protection systems have been selected to

APPENDIX C
TREATMENT FOR ELECTRIC SHOCK
Do not touch a person with bare hand unless the person is free from contact
with any electricity source.

FREE THE PERSON FROM CONTACT

1. Switch off the supply immediately or ask someone to do so.

2. Do not attempt to remove a person from contact with High Voltage unless
suitable articles insulated for the system High Voltage are used for this
purpose and Minimum Safety Clearance can be maintained.

3. When attempting to free a person from contact with Low or Extra Low
Voltage, use rubber gloves, boots, mat, or insulated stick. If these are
not available, use a loop of rope, cap or coat to drag the person free.
Whatever used should be dry and non-conducting.

AFTER RELEASE FROM ELECTRICITY

1. If the immediate vicinity is not safe, remove the injured person to a safe
place. Call 999 to seek help or follow the standing instruction for
calling emergency services applicable to the location.

2. If you are qualified in cardiopulmonary resuscitation (CPR), you should,


if appropriate, begin first aid while waiting for emergency medical help
to arrive.

With reference to the contents of the “Electric Shock” poster published by


Labour Department of the HKSAR, a guide on CPR is included below for reference.

Rescue the casualty and apply First Aid

Check the airway, breathing and pulse of the casualty. (Fig. 1)

Place casualty in the recovery position and call medical aid. (Fig. 2)

 If the casualty is NOT breathing and has NO pulse

Call medical aid, and then-

Start external cardiac compression – speed is essential (Fig. 3)

Feel for the lower half of the breastbone. Place the heel of your hand on
this part of the bone, keeping palm and fingers off the chest. Cover this
hand with the heel of the other hand.

With arms straight, rock forward, pressing down on the lower half of the
breastbone. Do this 30 times, at a rate of at least 100 strokes per minute.
Give the casualty two inflations every 30 compressions.

Start artificial ventilation

1. Check airway is not blocked. Remove loose fitting dentures, sweets etc.
from the casualty’s mouth.

2. Press head well back with one hand and pull the chin up with the other.
(Fig. 4)

3. Take a deep breath. Pinch casualty’s nostrils together with your


fingers. Seal your lips around his mouth and blow air steadily into
his lungs. Watch his chest rise. (Fig. 5)

4. Remove mouth and allow the chest to fall. (Fig. 6)

5. Give two breaths of artificial ventilation.

It takes about two minutes to complete 5 cycles of 30 compressions and 2


breaths. If the casualty is still not breathing and has no pulse, please
continue the cardiopulmonary resuscitation until the ambulanceman arrives
or the casualty has responses.

 If the casualty is NOT breathing but has pulse

Call medical aid and start artificial ventilation at a rate of 10 breaths


per minute. Check for a pulse after every 2 minutes. Place the casualty
in the recovery position when he starts breathing on his own.

Cover casualty with one blanket only and continue close observation. If
breathing stops again, turn casualty on his back and resume artificial
ventilation. If the pulse has also stopped, then perform external cardiac
compression as well.
