1.0 PURPOSE

The purpose is to describe for Descon and Its subcontractors the site
procedure for ensuring that the entire SHE controls that applis to the PMP
project are adhered to by all personnel at all time to help keep us incident
and injury free.

2.0 SCOPE

This procedure shall apply to Descon , its subcontractors, Vendors and


visitors at Plateau Maintenance Project.

3.0 RESPONSIBILTIES

Incident and Injury Free is primary responsibility of all levels of the


supervision, management and employees. Descon and its subcontractors
shall maximize the initiative of SHE related communication and training,
motivation and monitoring techniques.

The below section # describes the disciplinary action process to be used


by everyone on the PMP project to ensure that employees know what is
expected of them and adhere to the site SHE rules. This process provides
a general overview of the steps to be taken in the event of non
compliance to SHE rules.

4.0 RULE OF FAIR PLAY

PMP project has many SHE controls to help keep everyone safe and
incident and injury free. Many of these controls are mandatory and must
be adhered to by all personnel at all time.

Failure to comply with these controls, in particular the 12 keys to SHE is


unacceptable and the steps described in the “rule of fair play” should be
taken in the event of such behavior. These controls extend to the
supervisor responsible for the work execution along with the employee
responsible for the non conformance.

Rule of fair play is to determine the reason for unsafe behavior, which
can range from deliberate refusal to comply, to the incidental non
compliance caused by the lack of training, information or supervision.

Where non compliance is of an incidental or minor nature and has a low


potential to cause an incident or injury, employees will undergo further
training to reinforce good safety behavior. Punitive action will not be
taken against an employee who has not been trained to understand the
difference between acceptable and unacceptable behavior or if he is
within the probation period on the project unless an investigation reveals
that he has knowingly broken the rule on his own interest.

Where non compliance has a high potential to lead to an incident or


injury, or where the event was considered deliberate, the event will be
formally reported and recorded. Discipline for such acts will follow the
progressive steps via investigation as listed below

First Offence – Verbal Warning (Recorded)


Second Warning - Second Strike (Recorded)
Third Offence – 3rd strike (Removal from the PMP Project)

However Descon management still reserves the rights to permanently


remove anyone from the PMP project that it believes may cause harm to
themselves, their collogues or others or plant or equipment.

Transgressing or violating the following 11 Life Critical SHE rules will lead
to maximum appropriate disciplinary action being applied,

 Over speeding
 Working without PTW
 Not 100% tied of when expose to fall hazard
 Walking under the suspended Load
 Working under the influence of Alcohol
 Using cell phone while driving
 Not wearing the seat belt
 Smoking at undesignated Places especially in the live plant.
 Overriding the safety devise e.g., by passing crane computer
system.

 Bringing firearm, ammunition, explosive or lethal weapons on to the


work site except those approved for security personnel and as work
tool.
 Fighting, intimidation and violence

Where it is found that a supervisor set the condition for breaking the above
rule including providing direction for work to proceed with known non
compliance then they will face the maximum appropriate disciplinary action.

5.0 PROCESS

The following diagram outlines the disciplinary action process; the standard
applies the rule of fair play to determine the reason for the unsafe behavior or
non compliance.

The process will be used by the committee members as listed below in section
6.0 that are nominated by the management; these personnel are from
supervisor and management level. These personnel will be provided with
training to disseminate the knowledge of this procedure.
