    Note: (in company letterhead & must be signed)

    2 Xerox copy of signatory's 2 valid Gov't. I.D./Passport (2 copies)


    Note: with 3 specimen/signatures - side/or below portion.

    3 Original Copy of Computation of latest 3 months importation (2 copies ) /


    Note: (in company letterhead & must be signed)

    4 Original / Notarized Secretary's Certificate - ( 1 original copy & 1 xerox )


    (submit original copy for NAIA)
    (Please attach xerox copy - valid Gov't. ID of corporate secretary)
    Note: copy of Valid Gov't. ID of Corp. Sec. with 3 original specimen/signatures

    5. Latest/complete copy of General Information Sheet (GIS) filed with SEC.

    6. Copy of your company's PEZA Registration

    7. Copy of your company's latest PEZA Cert. of Tax Exemption

    8. Copy of SEC Registration

    9. Copy of valid BOC Certificate of Registration

    CLARK - GTSB REQUIREMENTS


    1 Affidavit of Undertaking (2 original copies )
    Note: (in company letterhead & must be signed)

    2 Xerox copy of signatory's 2 valid Gov't. I.D./Passport (2 copies)


    (or 1 Gov't. ID plus 1 Company ID)
    Note: with 3 specimen/signatures - side/or below portion.

    3 Computation of latest 3 months importation (2 copies per port) /

    4 Copy of your company's PEZA Registration

    5 Copy of your company's latest PEZA Cert. of Tax Exemption (97-01)

    6 Copy of valid BOC Certificate of Registration
