Re: Recommendation for Termination of Ms. Chris Marie Yosores – SHS Teacher

Sir,

This is to escalate to your office the poor performance rating of Ms. Chris Marie Yosores – one
of my teachers in the SHS Department, her non-compliance in the office of the Grade 11
Coordinator, and the students’ letter of complaints re Ms. Yosores’ poor teaching performance.
Attached hereinafter are:

 Photocopy of the log book that contains her tardiness and absenteeism (she is always late
of her Fil11a2 Class, 7:00 am to 8:30 am every MWF at room D)
 Teachers’ Evaluation Result evaluated by the Principal, Coordinator, Co-teacher and
students
 Grade 11 Coordinator’s report re Ms. Yosores’ non-compliance
 Letter of complaints sent by the students

Because of the foregoing, I hereby recommend for her termination. My office is very much
willing to coordinate with your office should you need any regarding her termination.
