JOINT AFFIDAVIT OF MARRIAGE

We, ALLEN GARCIA DELLOMES and NIKKI ANN M. DELLOMES, both of legal ages,
Filipinos and with residence and postal address at L7A Diego Silang, Fortunata Village, San
Isidro, Parañaque City, after having been duly sworn to in accordance with law, hereby depose
and say;

1. That we are the above-named affiants as well as the personal circumstances to whom
they pertain;

2. That we have been living together as husband and wife under the same roof,
continuously and without any interruption;

3. That we had contracted marriage at the Shrine of Jesus, The Way, The Truth and the
Life, Pasay City, Metro Manila and was solemnized by Rev. Fr. Socrates C. Montealto,
SSP on June 8, 2013;

4. That we are willingly and voluntarily executing this Joint Affidavit of Marriage to attest to
the truth of the foregoing statements and for whatever legal purposes this may serve
best.
