AFFIDAVIT OF SOLO PARENT

I, MATEA MACALINDONG STA. MARIA, of legal age, widow,

and a resident of Sitio Elinahan, Brgy. Luyahan, Lian, Batangas,
after having sworn to in accordance with law depose and state that:
1. I am the biological mother of STEPHANIE MACALINDONG
STA. MARIA and JONAS MACALINDONG STA. MARIA, who were
born on February 8, 2008 and October 17, 2011, respectively;
2. On November 25 2022, my husband and the father of my
children, JOEY STA. MARIA died a natural death;
3. I was thereafter left alone with the sole responsibility of
parenthood and with the guidance and help of my family and up to
the present, I am solely providing for the needs of my children;
4. I am executing this Affidavit to attest to the foregoing facts
and for the purpose of securing a Solo Parent ID and to receive the
benefits under the Solo Parents’ Welfare Act of 2000.
IN WITNESS WHEREOF, I have hereto set my hand this 10 th
