As per documents presented, interview conducted and visit to

the Head Office:

On General Labor Standards (GLS):

1. Non-payment of premium pay (1.25%) on overtime rendered


during ordinary day;
2. No lactation policy;
3. No Committee on Decorum and Investigation on Sexual
Harassment; and
4. No Family Welfare Committee/Programs.

On Occupational Safety and Health Standards (OSHS):

1. No trained Full time Nurse;


2. No part time Physician; and
3. No part time Dentist.

Notes: No updated Fire Safety Inspection Certificate


No written policy on conversion of unused 5 days
Service Incentive Leave.

On Security of Tenure (SOT):


F1 Hotel Manila has Service Agreements with the following
agencies:

1. ALL ABOUT PEOPLE, for its housekeeping, engineering,


food and beverage department deploying forty nine (49) sub-
contractor employees. It has capital stock of Three Million
Pesos (P 3,000,000.00) and D.O. 18-A Registration valid
until July 29, 2018.
2. UNLISOLUTIONS MANPOWER & EVENTS for additional
manpower services (engineering, housekeeping, driver of
hotel car, kitchen food and beverages department.) It has
deployed 44 sub-con employees and has D.O. 18-A
registration valid until Feb. 16, 2017, COC on General Labor
Standards and Occupational Safety and Health Standards
issued on Feb. 22, 2016, share capitalization of P
3,500,000.00.
3. PROJOBS Human Resource Marketing & Events Concept
Corporation, for additional manpower services (Kitchen,
Food and Beverage, CCTV Operator, Front Officer and
Concierge.) It has 48 sub-contractor employees.

*ON HIRING PRACTICE


- The above agencies pre-screen needed manpower for
deployment to herein principal that conduct final interview and
select final list of applicants for employment.
-Agencies conduct pre-employment orientation on its admin.
policies. Principal also conducts a three-day orientation on
rules and regulations, hotel standard operations and exposure
to various departments of the principal.

*ON MONITORING SUPERVISION/CONTROL


-Principal provides the workplace, tools and equipments for
office use of biometrics, cleaning materials for housekeeping
like vacuum cleaner, floor polishers, mask, gloves, etc.
-Agencies has supervisors assigned at the principal to monitor
attendance, coordinate with principal with regard to its
assigned task. Principal has supervisors to monitor and
evaluate the job performance of deployed agencies.
-Concerns of principal is directly coordinated with the agencies
supervisors like tardiness. Agencies then issued Memo on
Disciplinary Action upon report submitted by the principal.

*PAYMENT OF WAGES
-Agencies pay wages of its sub-con employees and remit
SSS/PhilHealth and Pag-IBIG premium benefits to its sub-con
employees.

*TERMINATION
-Upon interview with the agencies supervisors, agencies issue
termination notice upon recommendation of the principal.
