 เป็ นจำนวนเงิน ๑๐๐ USD โดยชำระเป็ นเงินสดในขั้นตอนการลงทะเบียน
ในวันงาน

๔. ข้อกำหนด

๔.๑ ข้าพเจ้า (ชื่อ – นามสกุล) ขอรับรองว่าข้าพเจ้ามีสิทธิ์ตามกฎหมายในการเป็ น

ตัวแทน (ชื่อหน่วยงาน / สถานศึกษา) เพื่อลงนามในใบสมัคร

๔.๒ ข้าพเจ้าขอรับรองว่า ข้าพเจ้าได้ศึกษา สอบถาม ทำความเข้าใจ กฎและกติกาการ

แข่งขันและเอกสารทั้งหมดที่เกี่ยวข้องได้เข้าใจถี่ถ้วนดีแล้วก่อนลงนามในเอกสารฉบับนี้ และ
ยินดีกำกับดูแลให้คณะผู้ฝึ กสอนปฏิบัติตามโดยไม่มีเงื่อนไข

๔.๓ ข้าพเจ้ายอมรับว่า การอธิบายกฎและกติการแข่งขันให้กับผู้เกี่ยวข้องกับทีมผู้เข้า

ประกวด ไม่ว่าจะเป็ นคณะผู้ฝึ กสอน ผู้บริหารหน่วยงาน นักเรียน ฯลฯ รวมถึงการติดตาม
ข่าวสารจากผู้จัดการประกวดนั้น เป็ นหน้าที่ของหัวหน้าคณะผู้ฝึ กสอน และยินดีกำกับดูแลให้
หัวหน้าคณะผู้ฝึ กสอนปฏิบัติตามโดยไม่มีเงื่อนไข

๔.๔ ข้าพเจ้าในนามตัวแทนทีมผู้เข้าประกวด ยินดีให้หน่วยงานของผู้เข้าประกวด สมัคร

เข้าเป็ นสมาชิกวิสามัญของสมาคมดนตรีและมาร์ชชิ่งอาร์ทสากล และจะชำระเงินรวมถึงส่ง
หลักฐานการชำระเงินให้เสร็จสิ้นภายในวันที่ วันที่ ๓๐ เมษายน ๒๕๖๕

STATEMENT OF RESPONSIBILITY, RELEASE OF LIABILITY, AND

AGREEMENT TO PARTICIPATE IN
WAMSB WORLD CHAMPIONSHIP 2022, BANGKOK, THAILAND

I, the undersigned, hereby warrant that I am of full age and have

the right to contract in my own name and in the name of the (name
of band). I have read and understand all rules and regulations of this
competition, prior to its execution, and I am fully familiar with the
contents thereof and willing to comply to Rules and Regulations of
WAMSB World Championship 2022 and WAMSB One World Judging
System and Guideline. I understand and hereby acknowledge that my
participation in the event is wholly voluntary. In consideration of
being allowed to participate in the Program, I hereby agree as
follows:

1) I hereby represent and warrant that all members of

aforementioned organization will be covered throughout their
participation in the event by a policy of comprehensive health and
accident insurance which provides coverage for injuries and illnesses
all members of aforementioned organization may sustain or
experience while inside or outside of Thailand, including in particular,
any country in which all members of aforementioned organization
will be residing and/or traveling while participating in the event. All
members of aforementioned organization agree to report to the
organizing committee at least sixty (60) days prior to my departure
any physical or mental condition all members of aforementioned
organization have which may require special medical attention or
accommodation during the event.

2) If any members of aforementioned organization should suffer an

injury or illness while participating in the event, or any other activity
associated with the event, I authorize the employees of organizing
committee to use their discretion to have the member treated at the
nearest health care facility and hereby give consent, and I take full
responsibility for that action and the costs associated with such care.

3) I understand that the organizing committee reserves the right to

make changes to the
event itinerary at any time and for any reason, with or without notice.

4) I understand and agree that throughout our participation in the

event, all members of aforementioned organization are responsible
for abiding by the laws of any country in which we are residing or
traveling and the policies and procedures of the organizing
committee and the host institution. The organizing committee
reserves the right to decline to accept or retain us in the event if, at
any time, our actions or general behavior impede the operation of
the event or the rights or welfare of any person. Similarly, if our
conduct violates the laws of any country in which we are residing or
traveling or any policy or procedure of the organizing committee or
of the host institution, I understand that we may be required to leave
the event in the sole reasonable discretion of the organizing
committee's employees, agents and representatives. I understand
and hereby acknowledge that all members of aforementioned
organization will be subject to discipline by organizing committee, as
well as by the host institution we attend or in whose facilities we
reside in connection with the event.

5) I consent to organizing committee’s use of our aforementioned

organization name, photograph, likeness, comments for use in the
organizing committee publications, including publicity or promotional
materials.

6) In consideration of our Band’s participation in the scheduled

venues of The WAMSB World Championship 2022, and for other good
and valuable consideration herein acknowledged as received, I, being
the person in charge, hereby grant to the organizing committee of the
WAMSB World Championship 2022 (“Photographer and
Videographer”), his/her heirs, legal representatives, and assigns, those
for whom Photographer and Videographer are acting, and those
acting with his/her authority and permission, the irrevocable and
unrestricted right and permission to take, use, re-use, publish, and
republish photographic portraits or pictures or footage of any and all
members of aforementioned organization or in which any and all
band members that may be included, in whole or in part, or
composite or distorted in character or form, without restriction as to
changes or alterations, in conjunction with the aforementioned
organization or a fictitious name, or reproductions thereof in color or
otherwise, made through any medium, and in any and all media now
or hereafter known for illustration, promotion, art, editorial,
advertising, trade, or any other purpose whatsoever. I also consent to
the use of any published matter in conjunction therewith.

7) I hereby waive any right that I, or any member, may have to

inspect or approve the finished product or products and the
advertising copy or other matter that may be used in connection
therewith or the use to which it may be applied.

8) I hereby release, discharge, and agree to save harmless

Photographer and Videographer, his/her heirs, legal representatives,
and assigns, and all persons acting under his/her permission or
authority or those for whom he/she is acting, from any liability by
virtue of any blurring, distortion, alteration, optical illusion, or use in
composite form, whether intentional or otherwise, that may occur or
be produced in the taking of said picture or in any subsequent
processing thereof, as well as any publication thereof, including
without limitation any claims for libel or violation of any right of
publicity or privacy.
